Description
Overview of the medicine
Neurega 100 mg Capsule contains Pregabalin, which is an antiepileptic and neuropathic pain medication. It is used to treat neuropathic pain, epilepsy (partial seizures), generalized anxiety disorder, and fibromyalgia.
Uses & Indications
Dosage
Adults
Initial dose of 75 mg twice daily or 50 mg three times daily, adjusted according to response and tolerability, up to a maximum of 300 mg twice daily (600 mg/day).
Elderly
Dose adjustment may be necessary in elderly patients due to potentially decreased renal function. Start with a lower dose.
Renal_impairment
Dose must be individualized based on creatinine clearance (CrCl). Lower doses are required for patients with CrCl < 60 mL/min.
How to Take
Neurega capsules should be taken orally, with or without food. Swallow the capsule whole with water.
Mechanism of Action
Pregabalin binds with high affinity to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system. This binding reduces the release of several neurotransmitters, including glutamate, norepinephrine, and substance P, thereby decreasing neuronal excitability and pain signaling.
Pharmacokinetics
Onset
Onset of analgesic effect often within 1 week for neuropathic pain. Anxiolytic effects may appear sooner.
Excretion
Excreted almost entirely unchanged in the urine (approximately 98%).
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration; peak plasma concentrations reached within 1.5 hours.
Metabolism
Negligible hepatic metabolism; does not induce or inhibit drug-metabolizing enzymes.
Side Effects
Contraindications
- Hypersensitivity to pregabalin or any component of the formulation.
- Patients with rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption.
Drug Interactions
ACE Inhibitors
Increased risk of angioedema when co-administered with pregabalin.
Opioids, Benzodiazepines, Alcohol
May enhance the central nervous system (CNS) depressant effects, increasing risk of drowsiness, sedation, and respiratory depression.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and rarely, coma. Management is supportive, and hemodialysis may be considered in severe cases.
Pregnancy & Lactation
Pregnancy Category C. Should only be used during pregnancy if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as it is excreted in human milk.

Lab Monitoring
- No routine lab monitoring is required. Renal function should be monitored, especially in elderly patients or those with pre-existing renal impairment, to adjust dosage.
Doctor Notes
- Emphasize the importance of gradual dose tapering to prevent withdrawal symptoms.
- Counsel patients on potential CNS depressant effects, especially with concomitant use of other CNS depressants (e.g., opioids, benzodiazepines, alcohol).
- Monitor for emergent psychiatric symptoms, including suicidal thoughts or behaviors.
Patient Guidelines
- Do not stop taking Neurega abruptly, as this may lead to withdrawal symptoms. Always consult your doctor for dose reduction.
- Avoid alcohol consumption while on this medication, as it may enhance dizziness and drowsiness.
- Report any unusual mood changes, suicidal thoughts, or signs of allergic reactions (e.g., swelling of face, lips, tongue) to your doctor immediately.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is close to the time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
