Description
Overview of the medicine
Pregabalin is used to treat neuropathic pain, fibromyalgia, and generalized anxiety disorder, and as an adjunctive therapy for partial onset seizures.
Uses & Indications
Dosage
Adults
Starting dose often 75 mg twice daily or 50 mg three times daily. May be increased up to 150-300 mg daily for neuropathic pain, and up to 600 mg daily for GAD or fibromyalgia, based on response and tolerability.
Elderly
Dosage adjustments may be required due to decreased renal function; careful monitoring is advised.
Renal_impairment
Dosage must be reduced significantly in patients with renal impairment (CrCl <60 mL/min). Specific dosage adjustments depend on creatinine clearance.
How to Take
Take orally with or without food. Swallow the capsule whole with water. Do not chew or crush.
Mechanism of Action
Pregabalin binds to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system, reducing the release of excitatory neurotransmitters. This leads to antinociceptive and anxiolytic effects.
Pharmacokinetics
Onset
Pain relief may be observed within 1 week of therapy for some conditions.
Excretion
Excreted predominantly unchanged in the urine.
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1.5 hours. Bioavailability is ≥90% and is not affected by food.
Metabolism
Negligibly metabolized in humans; less than 0.1% of the dose is metabolized.
Side Effects
Contraindications
- Hypersensitivity to pregabalin or any component of the formulation.
Drug Interactions
CNS depressants (e.g., opioids, benzodiazepines, alcohol)
May enhance the sedative effects and increase risk of respiratory depression. Concomitant use should be approached with caution.
Thiazolidinedione antidiabetic agents (e.g., pioglitazone)
May increase the risk of weight gain and peripheral edema when co-administered with pregabalin.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and rarely coma. Treatment is supportive and symptomatic. Hemodialysis may be considered in severe cases as pregabalin is dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. It is excreted into breast milk, so caution is advised during lactation; a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
