Description
Overview of the medicine
Neurega 25 mg capsule contains Pregabalin, which is primarily used to treat neuropathic pain, epilepsy (seizures), and generalized anxiety disorder. It works by affecting certain chemicals in the brain that send pain signals.
Uses & Indications
Dosage
Adults
Starting dose usually 75 mg twice daily (150 mg/day). May be increased to 150 mg twice daily, up to a maximum of 300 mg twice daily (600 mg/day) depending on indication and patient response.
Elderly
Dose adjustment may be necessary in elderly patients due to age-related decrease in renal function.
Renal_impairment
Dose reduction required based on creatinine clearance. For CrCl <30 mL/min, dose should be significantly reduced.
How to Take
Administer orally with or without food. Swallow the capsule whole with water.
Mechanism of Action
Pregabalin binds to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system, thereby reducing the release of several neurotransmitters, including glutamate, noradrenaline, and substance P. This action helps to reduce hyperexcitability of neurons responsible for pain and seizure activity.
Pharmacokinetics
Onset
Onset of action for pain relief can be within 1 week for some conditions.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and active tubular secretion.
Half life
Approximately 6.3 hours.
Absorption
Rapidly and well absorbed after oral administration, with peak plasma concentrations occurring within 1-1.5 hours.
Metabolism
Negligible metabolism in humans; approximately 98% is excreted unchanged in the urine.
Side Effects
Contraindications
- Hypersensitivity to pregabalin or any component of the formulation
Drug Interactions
Antidiabetic agents (e.g., rosiglitazone)
May increase the risk of weight gain and peripheral edema when co-administered with pregabalin.
CNS depressants (e.g., opioids, benzodiazepines, alcohol)
May potentiate the sedative effects of pregabalin, leading to increased drowsiness and impaired alertness.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and restlessness. Treatment is supportive and symptomatic. Hemodialysis may be useful in overdose if clinically indicated.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pregabalin is excreted into human milk; therefore, caution should be exercised when administered to a nursing woman.
