Description
Overview of the medicine
Nidazyl is a brand name for Metronidazole, an antibiotic and antiprotozoal medication used to treat various bacterial and parasitic infections.
Uses & Indications
Dosage
Adults
Varies depending on infection, typically 250-750 mg orally, 2-3 times daily for 5-10 days.
Elderly
Dosage adjustment may be required in patients with significant hepatic impairment; otherwise, adult doses are generally applicable.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment or dialysis, consult a physician.
How to Take
Oral tablets should be taken with food or milk to minimize gastrointestinal upset. Oral suspensions should be shaken well before use.
Mechanism of Action
Metronidazole enters bacterial and protozoal cells, where its nitro group is reduced to highly reactive cytotoxic compounds. These compounds bind to and damage DNA, inhibiting nucleic acid synthesis and leading to cell death.
Pharmacokinetics
Onset
Clinical effects typically observed within hours to days, depending on the infection.
Excretion
Mainly excreted in urine (60-80%) as unchanged drug and metabolites, with a small amount excreted in feces (6-15%).
Half life
Approximately 6-8 hours in adults.
Absorption
Rapid and almost complete absorption from the gastrointestinal tract after oral administration.
Metabolism
Extensively metabolized in the liver by oxidation and glucuronidation.
Side Effects
Contraindications
- •Hypersensitivity to metronidazole or other nitroimidazole derivatives.
- •First trimester of pregnancy (relative contraindication).
- •Concurrent use of disulfiram within the last 14 days.
Drug Interactions
Alcohol
Causes disulfiram-like reaction (nausea, vomiting, flushing, tachycardia).
Lithium
Increases serum lithium levels, leading to toxicity.
Warfarin
May potentiate anticoagulant effect, increasing risk of bleeding.
Cimetidine
May prolong metronidazole half-life, increasing risk of side effects.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, ataxia, and mild disorientation. Treatment is symptomatic and supportive; no specific antidote exists.
Pregnancy & Lactation
Not recommended during the first trimester of pregnancy. Use in second and third trimesters only if clearly needed. Metronidazole is excreted in breast milk; a decision should be made whether to discontinue nursing or the drug.
