Description
Overview of the medicine
Omeprazole is a proton pump inhibitor (PPI) that decreases the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
20-40 mg once daily, usually in the morning, for 4-8 weeks depending on the condition.
Elderly
No specific dose adjustment for elderly patients, but consider renal or hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Take the capsule whole with a glass of water, preferably in the morning before a meal. Do not chew or crush.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby suppressing gastric acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour, with maximal effect achieved after 2-3 days of continuous therapy.
Excretion
Approximately 80% of metabolites are excreted in the urine and 20% in the feces.
Half life
Plasma elimination half-life is approximately 0.5-1 hour, but the duration of acid suppression is much longer (up to 72 hours) due to irreversible binding.
Absorption
Rapidly absorbed after leaving the stomach, with peak plasma concentrations occurring 1-2 hours after administration of enteric-coated capsules.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to omeprazole or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
May increase INR and bleeding risk.
Nelfinavir
Reduced plasma levels of nelfinavir.
Clopidogrel
May reduce antiplatelet effect of clopidogrel.
