Overview of the medicine
Onepro 40 mg capsule contains Esomeprazole, a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD) and peptic ulcers.
Uses & Indications
Dosage
Adults
For most conditions, 20-40 mg once daily, usually taken at least one hour before a meal. Duration of treatment varies based on indication (e.g., GERD 4-8 weeks). For Zollinger-Ellison Syndrome, initial dose is typically 40 mg twice daily, adjusted as needed.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Take the capsule orally, usually at least one hour before a meal. Swallow the capsule whole with water; do not chew or crush it. If you have difficulty swallowing, the capsule can be opened and the pellets mixed with a small amount of water or soft food (e.g., apple sauce) and swallowed immediately.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase enzyme system (the 'proton pump') in the gastric parietal cells. This irreversible inhibition reduces gastric acid secretion by blocking the final step of acid production.

Excreted mainly in urine (approximately 80%) and feces (approximately 20%) as metabolites.

Approximately 1.5 hours (plasma half-life, but duration of acid suppression is much longer)
Absorption
Rapidly absorbed after oral administration, peak plasma concentration reached within 1-2 hours. Bioavailability is approximately 50-68% (single dose).
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Warfarin
Increased INR and prothrombin time; careful monitoring is recommended.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
May increase serum levels of methotrexate, leading to potential toxicity, especially at high doses.
HIV Antivirals (e.g., Nelfinavir, Atazanavir)
Reduced absorption and plasma concentrations of these drugs, potentially leading to loss of efficacy. Concomitant use with nelfinavir is contraindicated.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include lethargy, confusion, blurred vision, tachycardia, nausea, sweating, flushing, headache, and dry mouth. Treatment is symptomatic and supportive. Esomeprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Esomeprazole is likely excreted in human milk; use with caution in nursing mothers.
