Description
Overview of the medicine
Onlit 4 mg Oral Solution is an antiemetic medicine used to prevent and treat nausea and vomiting caused by chemotherapy, radiation therapy, and surgery. It belongs to a class of drugs called serotonin 5-HT3 receptor antagonists.
Uses & Indications
Dosage
Adults
Chemotherapy-induced nausea and vomiting: 4 mg (5 ml) orally twice daily for up to 5 days, starting before chemotherapy, or as directed by a physician. Post-operative nausea and vomiting: 4 mg (5 ml) orally 1 hour before anesthesia, followed by 4 mg (5 ml) 8 hourly for 2 further doses.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients over 75 years due to potential QT prolongation.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment (mild to moderate).
How to Take
Take Onlit 4 mg Oral Solution orally, with or without food, as directed by your doctor. Use a measuring cup or spoon provided with the solution to ensure accurate dosing.
Mechanism of Action
Ondansetron selectively blocks serotonin 5-HT3 receptors, which are found both centrally in the chemoreceptor trigger zone (CTZ) and peripherally on vagal nerve terminals in the gastrointestinal tract. By blocking these receptors, it prevents the initiation of the vomiting reflex.
Pharmacokinetics
Onset
Within 30 minutes of oral administration.
Excretion
Approximately 5% of the dose is excreted unchanged in the urine; metabolites are excreted via urine and feces.
Half life
Approximately 3-6 hours in adults, longer in elderly and patients with severe hepatic impairment.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract; peak plasma concentrations occur within 1.5-2 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP1A2, CYP2D6, CYP3A4).
Side Effects
Contraindications
- Known hypersensitivity to ondansetron or any component of the formulation.
- Concomitant use with apomorphine due to the risk of profound hypotension and loss of consciousness.
Drug Interactions
Tramadol
Ondansetron may reduce the analgesic effect of tramadol.
Apomorphine
Concomitant use may cause profound hypotension and loss of consciousness. Avoid co-administration.
Serotonergic drugs (e.g., SSRIs, SNRIs)
Increased risk of serotonin syndrome; monitor patients for symptoms.
Drugs that prolong QT interval (e.g., antiarrhythmics, antipsychotics)
Increased risk of QT prolongation and Torsade de Pointes; use with caution.
Storage
Store at controlled room temperature, 20°C to 25°C (68°F to 77°F). Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include transient blindness (lasting minutes to hours), severe constipation, hypotension, and vasovagal episodes with transient AV block. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Ondansetron should be used during pregnancy only if clearly needed. Excreted in breast milk; caution should be exercised when administered to a nursing mother. Consult your doctor before use.

- ECG monitoring (in patients with cardiac disease or risk factors for QT prolongation)
Doctor Notes
- Monitor ECG in patients with underlying cardiac conditions, electrolyte imbalances (hypokalemia, hypomagnesemia), or those taking other medications known to prolong the QT interval.
- Caution advised in patients with phenylketonuria due to aspartame content in some oral solutions.
- Consider alternative antiemetics or dose adjustments in patients with severe hepatic impairment.
