Overview of the medicine
Onlit 4 mg Tablet contains Ondansetron, an antiemetic used to prevent nausea and vomiting caused by chemotherapy, radiation therapy, and surgery.
Uses & Indications
Dosage
Adults
Chemotherapy-induced nausea and vomiting: 8 mg orally 30 minutes before chemotherapy, then 8 mg every 8 hours for 1-2 days. Radiotherapy-induced nausea and vomiting: 8 mg orally 1-2 hours before radiotherapy, then 8 mg every 8 hours. Postoperative nausea and vomiting: 16 mg orally one hour before anesthesia, or 4 mg IV immediately before anesthesia.
Elderly
No dosage adjustment needed for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment needed for patients with renal impairment.
How to Take
Onlit 4 mg Tablet can be taken with or without food. Follow your doctor's instructions carefully. Do not crush, chew, or break the tablet unless specifically instructed.
Mechanism of Action
Ondansetron is a selective 5-HT3 receptor antagonist. Serotonin (5-HT) receptors are found on vagal nerve terminals in the gut and in the chemoreceptor trigger zone (CTZ) in the brain. Chemotherapy, radiation therapy, and surgery release serotonin from enterochromaffin cells in the gut, which stimulates 5-HT3 receptors, initiating the vomiting reflex. Ondansetron blocks these 5-HT3 receptors, thereby preventing nausea and vomiting.
Pharmacokinetics
Onset
30 minutes to 2 hours
Excretion
About 5% of the dose is excreted unchanged in urine. The remaining is excreted as metabolites via urine and feces.
Half life
Approximately 3-5 hours (adults), 6-7 hours (elderly)
Absorption
Rapidly and completely absorbed from the GI tract after oral administration. Peak plasma concentrations are reached within 1.5 hours.
Metabolism
Extensively metabolized in the liver by hydroxylation, followed by glucuronide or sulfate conjugation.
Side Effects
Contraindications
- Hypersensitivity to ondansetron or any component of the formulation.
- Concomitant use with apomorphine.
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol.
Apomorphine
Concomitant use is contraindicated due to reports of profound hypotension and loss of consciousness.
QT-prolonging drugs
Use with caution; may increase the risk of QT interval prolongation.
Phenytoin, Carbamazepine, Rifampicin
May decrease ondansetron plasma concentrations.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose include severe constipation, hypotension, vasovagal episode with transient AV block. Management is supportive and symptomatic.

Pregnancy: Use only if clearly needed. Lactation: Not known whether ondansetron is excreted in human milk. Exercise caution when administered to a nursing mother.
