Description
Overview of the medicine
Ondansetron is an antiemetic used to prevent nausea and vomiting caused by cancer chemotherapy, radiation therapy, and surgery. It works by blocking the action of serotonin, a natural substance in the body that can cause nausea and vomiting.
Uses & Indications
Dosage
Adults
Highly emetogenic chemotherapy: 8 mg IV/IM immediately before chemotherapy, followed by 8 mg IV/IM 8 hours after the first dose, then 8 mg orally twice daily for 1-2 days. Postoperative nausea/vomiting: 4 mg IV/IM as a single dose.
Elderly
No dosage adjustment necessary for patients over 65 years. For patients over 75 years, initial IV dose should not exceed 16 mg.
Renal_impairment
No dosage adjustment necessary for patients with renal impairment.
How to Take
Onlit 8 mg injection is administered intravenously (IV) or intramuscularly (IM) by a healthcare professional. It should be diluted before IV infusion and given over a period of not less than 30 seconds, preferably over 2-5 minutes.
Mechanism of Action
Ondansetron is a selective 5-HT3 receptor antagonist. Serotonin 5-HT3 receptors are located both peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone (CTZ). By blocking these receptors, ondansetron prevents both central and peripheral stimulation that leads to nausea and vomiting.
Pharmacokinetics
Onset
Within 30 minutes following intravenous administration.
Excretion
Approximately 5-10% excreted unchanged in urine; remainder excreted as metabolites via urine and feces.
Half life
Approximately 3-6 hours in adults; prolonged in elderly and hepatic impairment.
Absorption
Rapidly and completely absorbed following oral administration. For injection, 100% bioavailability.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP2D6, CYP1A2, CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to ondansetron or any component of the formulation.
- Concomitant use of apomorphine.
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol.
Apomorphine
Concomitant use is contraindicated due to profound hypotension and loss of consciousness.
QT-prolonging drugs
Use with caution due to risk of QT prolongation (e.g., antiarrhythmics, antipsychotics).
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms of overdose include transient visual disturbances, severe constipation, hypotension, and vasovagal episodes with transient AV block. Management is supportive and symptomatic, with ECG monitoring due to QT prolongation risk.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Ondansetron is excreted in breast milk; caution should be exercised when administered to a nursing woman.
