Description
Overview of the medicine
Ondansetron is used to prevent nausea and vomiting caused by cancer chemotherapy, radiation therapy, and surgery.
Uses & Indications
Dosage
Adults
For chemotherapy: 8 mg orally twice daily. For radiation: 8 mg 1-2 hours before therapy. For PONV: 16 mg orally 1 hour before anesthesia.
Elderly
No general dosage adjustment required; monitor hepatic/renal function.
Renal_impairment
No dosage adjustment needed. For severe hepatic impairment, maximum 8 mg/day.
How to Take
Swallow the tablet whole with a glass of water, with or without food. Do not chew or crush.
Mechanism of Action
Ondansetron is a selective 5-HT3 receptor antagonist. It blocks serotonin, a natural substance in the body that can cause nausea and vomiting. It primarily works in the gastrointestinal tract and the chemoreceptor trigger zone (CTZ) in the brain.
Pharmacokinetics
Onset
Within 30 minutes for oral administration.
Excretion
Primarily excreted in urine (about 5-10% as unchanged drug) and feces (via bile).
Half life
Approximately 3-5 hours.
Absorption
Rapidly and completely absorbed from the GI tract after oral administration, but undergoes first-pass metabolism. Bioavailability is about 60%.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP1A2, CYP2D6, CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to ondansetron or any component of the formulation.
- Concomitant use with apomorphine.
Drug Interactions
Apomorphine
Increased risk of severe hypotension and loss of consciousness.
QT-prolonging drugs
Increased risk of QT interval prolongation, leading to potential arrhythmias.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include visual disturbances, severe constipation, hypotension, and vasovagal episodes with transient AV block. Treatment is symptomatic and supportive.

Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted into breast milk in animals; exercise caution when administered to a nursing woman.
