Description
Overview of the medicine
Ofloxacin is a synthetic broad-spectrum antibacterial agent of the fluoroquinolone class. It is active against a wide range of gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
Typically 400 mg once daily or 200 mg twice daily, depending on the infection. Duration varies from 7-14 days.
Elderly
Dose adjustment may be necessary in elderly patients with reduced renal function.
Renal_impairment
Initial dose 400 mg, then subsequent doses adjusted based on creatinine clearance (e.g., 200 mg every 24-48 hours).
How to Take
Take the tablet orally with water, with or without food. Do not chew or crush. Maintain adequate hydration.
Mechanism of Action
Inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination.

Excretion
Primarily excreted unchanged in urine via glomerular filtration and tubular secretion.

Approximately 4-7 hours (prolonged in renal impairment).
Absorption
Rapidly and well absorbed from the GI tract (bioavailability ~90-100%). Peak plasma concentrations reached within 1-2 hours.
Metabolism
Minimally metabolized in the liver; about 5% is metabolized.
Side Effects
Contraindications
- •Hypersensitivity to ofloxacin or other fluoroquinolones
- •History of tendinitis or tendon rupture associated with quinolone use
- •Epilepsy or other seizure disorders
Drug Interactions
Warfarin
May enhance anticoagulant effects, requiring INR monitoring.
Theophylline
Increased risk of theophylline toxicity; monitor levels.
Antacids/Iron/Zinc
Reduced absorption of ofloxacin; administer 2-4 hours apart.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include CNS effects (confusion, dizziness, seizures), GI disturbances. Treatment is symptomatic and supportive, including gastric lavage and hydration.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to fetus. Excreted in breast milk, discontinue nursing or the drug.
