Overview of the medicine
Primocef 1.5 gm Injection contains Ceftriaxone, a third-generation cephalosporin antibiotic, used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Typical adult dose is 1-2 gm intravenously (IV) or intramuscularly (IM) once daily or in two divided doses. Max 4 gm/day.
Elderly
No specific dose adjustment for elderly with normal renal and hepatic function.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment (CrCl <10 mL/min), dose should not exceed 2 gm daily.
How to Take
Administered by slow intravenous (IV) injection over 2-4 minutes, or by deep intramuscular (IM) injection. IV infusion over 30 minutes also possible.
Mechanism of Action
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Onset within 1 hour after administration.
Excretion
Biliary and renal excretion.

Approximately 6-9 hours.

Side Effects
Contraindications
- Hypersensitivity to ceftriaxone, other cephalosporins, or any component of the formulation
- Hyperbilirubinemic neonates, especially premature infants
- Concomitant use with calcium-containing IV solutions in neonates
Drug Interactions
Aminoglycosides
Synergistic effect, but should not be mixed in the same syringe.
Oral anticoagulants
May increase the anticoagulant effect, requiring INR monitoring.
Calcium-containing solutions
Concomitant use is contraindicated in neonates due to precipitation risk.
Loop diuretics (e.g., Furosemide)
May increase ceftriaxone concentrations.
