Overview of the medicine
Primocef 500 mg Tablet contains Cefixime, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections in various parts of the body.
Uses & Indications
Dosage
Adults
The usual adult dose is 500 mg once daily or 250 mg twice daily. For uncomplicated gonorrhea, a single 400 mg or 800 mg dose is often used (use appropriate strength if available). The duration of treatment depends on the infection, typically 7-14 days.
Elderly
No specific dose adjustment is required unless there is significant renal impairment.
Renal_impairment
For creatinine clearance 21-60 mL/min, dose should be 75% of standard dose. For creatinine clearance <20 mL/min, dose should be 50% of standard dose.
How to Take
Primocef 500 mg Tablet can be taken with or without food. To minimize gastrointestinal upset, it may be taken with food. Swallow the tablet whole with water; do not crush or chew.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations reached 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (about 50-60% of absorbed dose) and some in bile/feces.
Half life
Approximately 3-4 hours.
Absorption
Approximately 30-50% orally absorbed. Food can delay time to peak concentration but does not significantly alter the extent of absorption.
Metabolism
Not extensively metabolized in the liver; approximately 10% is metabolized.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime or other cephalosporins.
- History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase Cefixime bioavailability.
Carbamazepine
Concomitant administration may increase carbamazepine plasma concentrations.
Oral contraceptives
Some antibiotics, including cephalosporins, may reduce the effectiveness of oral contraceptives. Advise patients to use a backup contraceptive method.
Warfarin and other anticoagulants
May increase prothrombin time (PT) and INR, increasing the risk of bleeding. Close monitoring is recommended.

Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.

In case of overdose, Cefixime is generally well tolerated. Gastric lavage may be indicated; otherwise, no specific antidote exists. Treatment should be symptomatic and supportive.

Pregnancy Category B. Animal reproduction studies have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; use with caution in breastfeeding mothers.
