Description
Overview of the medicine
Primocef 750 mg Injection contains Cefuroxime Sodium, a second-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections in various parts of the body.
Uses & Indications
Dosage
Adults
Most infections: 750 mg IV/IM every 8 hours. Severe infections: 1.5 g IV every 8 hours. Gonorrhea: 1.5 g IM as a single dose. Surgical prophylaxis: 1.5 g IV pre-operatively, followed by 750 mg IV/IM every 8 hours for up to 24-48 hours.
Elderly
Dosage adjustments may be necessary in elderly patients with significantly impaired renal function.
Renal_impairment
Dosage reduction required based on creatinine clearance. For CrCl 10-20 mL/min, 750 mg every 12 hours. For CrCl <10 mL/min, 750 mg every 24 hours. For hemodialysis, 750 mg at the end of each dialysis session.
How to Take
For intramuscular (IM) use, reconstitute with sterile water for injection and inject deeply into a large muscle. For intravenous (IV) use, reconstitute and administer slowly over 3-5 minutes or infuse over 30-60 minutes.
Mechanism of Action
Cefuroxime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), disrupting the peptidoglycan layer formation, leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration.
Excretion
Primarily via renal excretion (glomerular filtration and tubular secretion).
Half life
Approximately 1.2 to 1.6 hours (adults with normal renal function).
Absorption
Administered intravenously or intramuscularly, resulting in rapid and complete absorption. Oral form (Cefuroxime axetil) is absorbed and hydrolyzed to cefuroxime.
Metabolism
Not significantly metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime or any other cephalosporin antibiotics
- History of severe hypersensitivity (e.g., anaphylactic reaction) to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems, monobactams)
Drug Interactions
Probenecid
Increases cefuroxime plasma concentrations and prolongs its half-life by decreasing renal tubular secretion.
Oral anticoagulants
May potentiate the anticoagulant effect, increasing INR.
Aminoglycosides and potent diuretics (e.g., furosemide)
May increase the risk of nephrotoxicity.
Storage
Store below 30°C. Protect from light and moisture. Reconstituted solution should be used immediately or stored for a limited period as per manufacturer guidelines.
Overdose
Symptoms of overdose include convulsions, encephalopathy, and other neurological sequelae. Management involves supportive care and hemodialysis to remove the drug from the bloodstream.
Pregnancy & Lactation
Pregnancy Category B. Cefuroxime crosses the placenta. Use with caution during pregnancy; benefits should outweigh risks. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for potential side effects.

Lab Monitoring
- Renal function tests (BUN, creatinine) during prolonged therapy or in patients with renal impairment.
- Liver function tests (ALT, AST) periodically.
- Complete blood count (CBC) to monitor for eosinophilia or other hematologic changes.
Doctor Notes
- Careful history taking regarding allergic reactions to beta-lactam antibiotics is crucial before administration.
- Monitor renal function, especially in elderly or renally impaired patients.
- Be aware of potential for C. difficile-associated diarrhea, even weeks after treatment cessation.
Patient Guidelines
- Complete the full course of antibiotics as prescribed, even if symptoms improve.
- Do not save medication for future use or share it with others.
- Report any severe or persistent side effects to your doctor immediately.
Missed Dose Advice
If a dose is missed, administer it as soon as remembered, unless it's nearly time for the next scheduled dose. Do not double the dose.
Driving Precautions
