Description
Overview of the medicine
Prioxon (Piroxicam) is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and stiffness caused by various inflammatory conditions, including arthritis.
Uses & Indications
Dosage
Adults
Typically 20 mg once daily, or 10 mg twice daily. For acute gout, 40 mg once daily for 5-7 days.
Elderly
Consider lower doses and careful monitoring due to increased risk of adverse effects.
Renal_impairment
Use with caution; dose reduction may be necessary in severe impairment.
How to Take
Take orally, preferably with or after food to minimize gastrointestinal upset. Swallow whole with water.
Mechanism of Action
Inhibits cyclooxygenase (COX-1 and COX-2) enzymes, reducing prostaglandin synthesis, thereby exerting anti-inflammatory, analgesic, and antipyretic effects.
Pharmacokinetics
Onset
Analgesic effect begins within 1 hour, anti-inflammatory effect may take 7-12 days.
Excretion
Primarily excreted in urine (about 2/3) and feces (about 1/3) as metabolites.
Half life
Approximately 50 hours.
Absorption
Well absorbed orally; peak plasma concentrations reached in 3-5 hours.
Metabolism
Extensively metabolized in the liver by hydroxylation.

Contraindications

Drug Interactions
Lithium
Increased plasma lithium levels.
Warfarin
Increased risk of bleeding.
Methotrexate
Increased methotrexate toxicity.
ACE inhibitors/Diuretics
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include lethargy, drowsiness, nausea, vomiting, and epigastric pain. Management is symptomatic and supportive; gastric lavage or activated charcoal may be considered if ingestion was recent.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to risk of premature closure of the fetal ductus arteriosus. Use with caution in other trimesters if benefits outweigh risks. Small amounts are excreted in breast milk; consult a doctor before use during lactation.
