Description
Overview of the medicine
Ciprofloxacin ophthalmic solution is an antibiotic eye drop used to treat bacterial infections of the eye, such as bacterial conjunctivitis and corneal ulcers.
Uses & Indications
Dosage
Adults
Bacterial Conjunctivitis: 1-2 drops into the affected eye(s) every 4-6 hours for 5-7 days. Severe infections: 2 drops every 15 minutes for 6 hours, then 2 drops every 30 minutes for the remainder of the first day; then 2 drops every hour on day 2, and 2 drops every 4 hours from day 3-14.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for ophthalmic administration due to minimal systemic absorption.
How to Take
For ophthalmic use only. Tilt head back, pull down lower eyelid to form a pouch, instill the prescribed number of drops without touching the eye or eyelid with the dropper tip. Close the eye gently and apply light pressure to the inner corner of the eye for 1-2 minutes to minimize systemic absorption.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.
Pharmacokinetics
Onset
Rapid onset of action, with bacterial eradication beginning within minutes to hours of administration.
Excretion
Systemically absorbed ciprofloxacin is primarily excreted renally, both as unchanged drug and metabolites.
Half life
Not well characterized for ophthalmic use due to minimal systemic absorption. Systemic half-life is approximately 3-5 hours.
Absorption
Minimal systemic absorption occurs following topical ophthalmic administration. Ocular tissue concentrations are achieved readily.
Metabolism
Minimal metabolism occurs within ocular tissues. If systemically absorbed, it is primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Ciprofloxacin, any other fluoroquinolone, or any component of the medication
- Viral or fungal eye infections (unless specifically indicated by a physician)
Drug Interactions
None clinically significant with ophthalmic administration
Due to minimal systemic absorption, drug interactions commonly seen with systemic ciprofloxacin (e.g., with Warfarin, Theophylline) are not expected with ophthalmic use.
Storage
Store below 30°C. Protect from direct light and moisture. Do not freeze. Keep the bottle tightly closed when not in use.
Overdose
Overdose is unlikely with topical ophthalmic administration. If accidental ingestion occurs, seek medical advice, although systemic toxicity from ophthalmic drops is generally low.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing mother, as ciprofloxacin is excreted in human milk after systemic administration. Minimal systemic absorption with ophthalmic use reduces this concern.

Lab Monitoring
- No routine lab monitoring is required for ophthalmic use of Ciprofloxacin.
Doctor Notes
- Advise patients regarding the potential for white precipitates in corneal defects and reassure them that these usually resolve with continued use.
- Emphasize meticulous administration technique to prevent contamination of the dropper tip and ensure optimal drug delivery.
- Monitor patient compliance closely and stress the importance of completing the full course of treatment to prevent therapeutic failure and development of antimicrobial resistance.
Patient Guidelines
- Do not wear contact lenses during treatment for bacterial eye infections.
- Wash hands thoroughly before and after applying the eye drops.
- Avoid touching the dropper tip to the eye, eyelid, or any other surface to prevent contamination.
- Complete the full course of treatment, even if symptoms improve, to ensure eradication of infection and prevent resistance.
- If using other eye medications, wait at least 5-10 minutes between applications.
Missed Dose Advice
Apply the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue your regular dosing schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Procin-03 Eye Drop may cause temporary blurred vision or other visual disturbances immediately after administration. Patients should be advised not to drive or operate machinery until their vision clears.
Lifestyle Advice
- Avoid rubbing eyes, as this can worsen irritation or spread infection.
- Maintain good ocular hygiene to prevent reinfection.
- Avoid sharing eye drops with others.
