Description
Overview of the medicine
Ranzith (Azithromycin) is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and sexually transmitted diseases. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
500 mg once daily for 3 days OR 500 mg on Day 1, then 250 mg once daily for 4 days for respiratory/skin infections. For Chlamydia: 1g single dose.
Elderly
No dosage adjustment needed, but monitor for QT prolongation.
Renal_impairment
No dosage adjustment necessary for mild to moderate impairment (CrCl >10 mL/min). Exercise caution in severe impairment.
How to Take
Take orally, with or without food. Taking with food may decrease gastrointestinal upset.
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting protein synthesis. This results in bacteriostatic effects, though it can be bactericidal at higher concentrations or against highly susceptible organisms.

Onset
Clinical effects typically observed within 2-3 days.
Excretion
Primarily excreted unchanged in bile; a small amount is excreted in urine.
Half life
Terminal elimination half-life is 2-4 days.
Absorption
Rapidly absorbed from the gastrointestinal tract, but bioavailability is approximately 37% due to extensive tissue distribution.

Primarily metabolized in the liver, but extensive metabolism does not significantly reduce serum concentration.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Warfarin
Increased anticoagulant effect; monitor INR.
Nelfinavir
Increased azithromycin serum concentrations; no dosage adjustment necessary for azithromycin, but monitor for side effects.
Antacids (aluminum and magnesium-containing)
May reduce peak serum levels of azithromycin. Administer azithromycin at least 1 hour before or 2 hours after antacids.

Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.

Symptoms of overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management is symptomatic and supportive. Gastric lavage and supportive measures are indicated.
