Overview of the medicine
Reset 10 mg Injection is an antiemetic and prokinetic agent used to treat nausea, vomiting, and certain gastrointestinal motility disorders. It works by blocking dopamine receptors in the brain and increasing the movement of food through the digestive system.
Uses & Indications
Dosage
Adults
Typically 10 mg IV or IM, administered up to 3-4 times daily. Doses should not exceed 0.5 mg/kg/day.
Elderly
Elderly patients may require dose reduction due to decreased renal function and increased susceptibility to adverse effects. Start with a lower dose.
Renal_impairment
For creatinine clearance (CrCl) < 40 mL/min, reduce the dose by 50%. For CrCl < 10 mL/min, reduce the dose by 75%.
How to Take
Reset 10 mg Injection can be administered intravenously (IV) or intramuscularly (IM). For IV administration, inject slowly over at least 1-2 minutes to avoid transient but intense feelings of anxiety and restlessness.
Mechanism of Action
Reset acts primarily as a dopamine D2 receptor antagonist, both centrally in the chemoreceptor trigger zone (CTZ) and peripherally in the gastrointestinal tract. It also has weak serotonin 5-HT3 receptor antagonist and 5-HT4 receptor agonist properties, which contribute to its prokinetic effect by increasing acetylcholine release in the enteric nervous system, thereby enhancing gastric emptying and intestinal transit.

Excretion
Excreted primarily by the kidneys (approximately 85% of a dose within 72 hours), with about 20% excreted as unchanged drug and the remainder as metabolites.

Absorption
Rapidly and completely absorbed after intramuscular (IM) administration; immediate onset after intravenous (IV) administration.
Metabolism
Undergoes hepatic metabolism, primarily by sulfation and glucuronidation. Cytochrome P450 involvement is minimal.
Side Effects
Contraindications
- Hypersensitivity to the active substance or any excipients.
- Gastrointestinal hemorrhage, mechanical obstruction, or perforation.
- Patients with pheochromocytoma, due to the risk of hypertensive crisis.
- Patients with epilepsy, as it may increase seizure frequency.
- Patients with a history of tardive dyskinesia from neuroleptics or metoclopramide.
- Use in children less than 1 year of age due to increased risk of extrapyramidal disorders.
Drug Interactions
SSRIs
Increased risk of serotonin syndrome when co-administered.
Levodopa
Reset may antagonize the effect of levodopa on Parkinson's disease, and vice-versa.
MAO Inhibitors
Increased risk of hypertensive crisis if given with pheochromocytoma.
Anticholinergics and Opioid Analgesics

Symptoms of overdose may include drowsiness, disorientation, extrapyramidal reactions (e.g., muscle spasms, involuntary movements), and cardiovascular effects such as bradycardia. Management is primarily supportive and symptomatic. Anticholinergic drugs or benzodiazepines may be used to control extrapyramidal symptoms. Monitor vital signs and provide general supportive care.

Reset should be used during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. It is excreted into breast milk and may cause adverse effects in the breastfed infant; therefore, use is generally not recommended during lactation or a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
