Overview of the medicine
Reset 500 mg Tablet contains Paracetamol, a widely used medication for relieving mild to moderate pain and reducing fever. It works by inhibiting prostaglandin synthesis in the central nervous system.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg every 4-6 hours as needed. Maximum 4000 mg (4g) in 24 hours.
Elderly
No specific dose reduction usually required, but caution in frail patients or those with impaired renal/hepatic function. Consult doctor.
Renal_impairment
Dose adjustment may be necessary in severe renal impairment (creatinine clearance <10 ml/min). Consult doctor.
How to Take
Swallow the tablet whole with water. Can be taken with or without food. Do not exceed the recommended dose.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system, increasing the pain threshold by inhibiting cyclooxygenase (COX) enzymes, mainly COX-2, involved in prostaglandin synthesis. It also has antipyretic effects due to its action on the hypothalamic heat-regulating center.

Onset
Within 30-60 minutes for analgesia and antipyresis.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates; less than 5% is excreted unchanged.
Half life
1 to 3 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract; peak plasma concentrations occur within 30-60 minutes.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation, and a minor pathway involves cytochrome P450 leading to a toxic metabolite (NAPQI) which is detoxified by glutathione.
Side Effects
Contraindications

Drug Interactions
Alcohol
Increased risk of liver toxicity, especially with chronic heavy alcohol consumption.
Warfarin
May increase the anticoagulant effect, leading to increased bleeding risk, especially with prolonged daily use.
Phenytoin, Carbamazepine, Barbiturates
May enhance paracetamol's metabolism, increasing risk of hepatotoxicity.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, abdominal pain, and liver damage (which may manifest days later). Severe overdose can lead to hepatic necrosis, renal tubular necrosis, and death. Treatment involves gastric lavage if recent, activated charcoal, and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation at recommended doses, but always consult a doctor before use.
