Overview of the medicine
Ritch 60 mg Tablet contains Etoricoxib, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation caused by various conditions.
Uses & Indications
Dosage
Adults
Osteoarthritis: 60 mg once daily. Rheumatoid Arthritis, Ankylosing Spondylitis: 90 mg once daily. Acute Gouty Arthritis: 120 mg once daily (maximum 8 days). Acute Pain: 90 mg or 120 mg once daily (maximum 7 days).
Elderly
No dose adjustment generally required, but caution due to potential renal, hepatic, or cardiovascular impairment.
Renal_impairment
Avoid in severe renal impairment (CrCl <30 mL/min). Use with caution in moderate impairment.
How to Take
Oral. Can be taken with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Etoricoxib selectively inhibits cyclooxygenase-2 (COX-2), an enzyme responsible for the synthesis of prostaglandins, which mediate pain, inflammation, and fever.
Pharmacokinetics
Onset
Approximately 1 hour for analgesic effect.
Excretion
Excreted primarily via urine (70%) and feces (20%) as metabolites.
Half life
Approximately 22 hours.
Absorption
Well absorbed orally, with an absolute bioavailability of approximately 100%. Peak plasma concentration is reached in approximately 1 hour.
Metabolism
Extensively metabolized by cytochrome P450 enzymes (mainly CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to etoricoxib or any excipient
- •Active peptic ulceration or gastrointestinal bleeding
- •Severe hepatic impairment
- •Severe renal impairment (CrCl <30 mL/min)
- •Congestive heart failure (NYHA Class II-IV)
- •Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- •Pregnancy and breastfeeding
Drug Interactions
Lithium
Increased plasma lithium levels, leading to potential toxicity.
Warfarin
Increased risk of bleeding due to enhanced anticoagulant effect.
Diuretics
Reduced diuretic and antihypertensive effect, increased risk of renal impairment.
Methotrexate
Increased methotrexate toxicity due to reduced renal clearance.
ACE inhibitors/ARBs
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Symptomatic and supportive treatment is recommended. Gastric lavage or activated charcoal may be considered within an hour of ingestion.
Pregnancy & Lactation
Should be avoided during pregnancy, especially in the third trimester due to risk of premature closure of ductus arteriosus. Not recommended during breastfeeding.
