Description
Overview of the medicine
Roxagen 100 mg tablet is a non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain, inflammation, and fever. It works by inhibiting the production of certain natural substances in the body that cause inflammation and pain.
Uses & Indications
Dosage
Adults
100 mg orally, twice daily. May be increased to 150 mg twice daily if necessary, under medical supervision.
Elderly
Lower doses may be considered, usually 50 mg twice daily, due to increased risk of adverse effects. Monitor renal function.
Renal_impairment
Use with caution. Dosage reduction may be necessary in patients with moderate to severe renal impairment (creatinine clearance < 30 mL/min). Avoid in severe renal failure.
How to Take
Take the tablet orally with food or milk to minimize gastrointestinal discomfort. Do not crush or chew the tablet. Swallow whole with a glass of water.
Mechanism of Action
Roxacene acts by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, which are responsible for the synthesis of prostaglandins. Prostaglandins are mediators of inflammation, pain, and fever. By reducing prostaglandin synthesis, Roxacene exerts its analgesic, anti-inflammatory, and antipyretic effects.
Pharmacokinetics
Onset
Analgesic effect observed within 30-60 minutes.
Excretion
Excreted mainly through urine (approximately 60%) and feces (approximately 30%) as metabolites and a small portion as unchanged drug.
Half life
Plasma elimination half-life is approximately 4-6 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, peak plasma concentrations usually achieved within 1-2 hours after oral administration.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes (e.g., CYP2C9) to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to Roxacene, aspirin, or other NSAIDs.
- •History of gastrointestinal bleeding or peptic ulcer disease.
- •Severe heart failure (NYHA Class III-IV).
- •Severe renal or hepatic impairment.
- •Third trimester of pregnancy.
- •Coronary artery bypass graft (CABG) surgery.
Drug Interactions
Lithium
Increased plasma lithium levels, leading to toxicity.
Methotrexate
Increased plasma methotrexate levels and toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration and bleeding.
Diuretics and ACE inhibitors
Reduced antihypertensive effect and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store in a cool, dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, nausea, vomiting, epigastric pain, lethargy, and gastrointestinal bleeding. In rare cases, hypertension, acute renal failure, respiratory depression, and coma may occur. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered within the first hour of ingestion.
Pregnancy & Lactation
Pregnancy Category C (first and second trimesters); avoid in third trimester (Category D due to risk of premature closure of ductus arteriosus). Use during lactation only if the potential benefit justifies the potential risk to the infant, as NSAIDs can pass into breast milk.
