Description
Overview of the medicine
Roxagen 70 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) primarily used for the relief of pain and inflammation associated with various conditions such as osteoarthritis, rheumatoid arthritis, and acute pain. It works by selectively inhibiting the cyclooxygenase-2 (COX-2) enzyme.
Uses & Indications
Dosage
Adults
Osteoarthritis: 70 mg once daily. Rheumatoid Arthritis: 70 mg once daily. Acute Pain/Dysmenorrhea: 70 mg once daily, or 70 mg twice daily if needed, for a short duration.
Elderly
No specific dose adjustment is generally required, but caution is advised due to potential age-related renal/hepatic impairment. Start with the lowest effective dose.
Renal_impairment
Mild to moderate renal impairment: no dose adjustment needed. Severe renal impairment (CrCl < 30 mL/min): use with caution; consider lower doses.
How to Take
Roxagen 70 mg Tablet should be taken orally, with or without food. It is recommended to take it with food or milk if stomach upset occurs. Swallow the tablet whole with a glass of water, do not crush or chew.
Mechanism of Action
Roxagen selectively inhibits cyclooxygenase-2 (COX-2), an enzyme responsible for the synthesis of prostaglandins involved in inflammation, pain, and fever. By inhibiting COX-2, it reduces the production of these inflammatory mediators, thereby exerting its analgesic, anti-inflammatory, and antipyretic effects.
Pharmacokinetics
Onset
Pain relief typically begins within 30-60 minutes.
Excretion
Excreted mainly in the urine (approximately 50-60%) and feces (approximately 25-40%) as metabolites.
Half life
Approximately 8-12 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, reaching peak plasma concentrations within 2-4 hours after oral administration.
Metabolism
Primarily metabolized in the liver via cytochrome P450 enzymes (e.g., CYP2C9).
Side Effects
Contraindications
- •Hypersensitivity to Roxagen, sulfonamides, or any components of the formulation
- •History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- •Active gastrointestinal bleeding or peptic ulceration
- •Severe heart failure (NYHA Class III-IV)
- •Severe renal impairment (CrCl < 30 mL/min) or severe hepatic impairment
- •Third trimester of pregnancy
Drug Interactions
Lithium
Increased plasma lithium levels, leading to toxicity. Monitor lithium levels.
Diuretics
Reduced diuretic and antihypertensive effects. Monitor for signs of renal impairment.
Methotrexate
Increased plasma methotrexate levels, potentially increasing toxicity. Use with caution.
Fluconazole, other CYP2C9 inhibitors
May increase Roxagen plasma concentrations. Consider lower Roxagen dose.
Warfarin and other oral anticoagulants
Increased risk of bleeding. Close monitoring of INR/PT is required.
ACE inhibitors and Angiotensin II Receptor Blockers (ARBs)
Reduced antihypertensive effect and increased risk of renal impairment. Monitor blood pressure and renal function.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of NSAID overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Rarely, hypertension, acute renal failure, respiratory depression, and coma may occur. Management is primarily supportive and symptomatic. Gastric lavage and/or activated charcoal may be considered within 4 hours of ingestion.
Pregnancy & Lactation
Pregnancy: Use of NSAIDs, including Roxagen, during the third trimester of pregnancy is contraindicated due to the risk of premature closure of the fetal ductus arteriosus. Use during other trimesters only if the potential benefit justifies the potential risk to the fetus. Lactation: Roxagen is excreted in breast milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
