Cefuroxime Axetil is a broad-spectrum cephalosporin antibiotic used to treat a wide range of bacterial infections. The 100 mg suspension is primarily formulated for pediatric use.
Uses & Indications
Dosage
Adults
Generally, 250 mg to 500 mg twice daily, depending on the infection. For suspension, dosage is typically for pediatric use.
Elderly
Similar to adult dosage, but dose adjustment may be necessary in case of renal impairment.
Renal_impairment
Dosage interval should be prolonged or dose reduced in patients with significant renal impairment (creatinine clearance < 30 mL/min).
How to Take
Take orally with food for optimum absorption. Shake the bottle well before each use. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Cefuroxime Axetil is a prodrug that is hydrolyzed to cefuroxime. Cefuroxime is a bactericidal agent that works by inhibiting bacterial cell wall synthesis, leading to cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are usually achieved 2-3 hours after administration with food.
Excretion
Mainly excreted unchanged in the urine by both glomerular filtration and tubular secretion. Approximately 50% of the dose is excreted within 12 hours.
Half life
Approximately 1 to 1.5 hours in healthy adults.
Absorption
Rapidly absorbed from the gastrointestinal tract after oral administration and rapidly hydrolyzed to cefuroxime. Absorption is enhanced when taken with food.
Metabolism
Cefuroxime axetil is extensively hydrolyzed by esterases in the intestinal mucosa and blood to its active form, cefuroxime. Cefuroxime itself is not metabolized.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime or to any of the excipients.
- Known hypersensitivity to cephalosporin antibiotics.
- History of severe hypersensitivity (e.g., anaphylactic reaction) to any other type of beta-lactam antibacterial agent (e.g., penicillins, monobactams and carbapenems).
Drug Interactions
Probenecid
Concomitant administration of probenecid increases the AUC of cefuroxime by about 50%, prolonging its elimination.
Oral Anticoagulants
May increase INR in patients taking oral anticoagulants. Monitor INR closely.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; advise additional contraceptive methods.
Antacids/H2-receptor antagonists
May decrease the bioavailability of cefuroxime axetil, potentially reducing its effectiveness. Administer at least 2 hours apart.
Storage
Store the dry powder below 30°C, protected from light and moisture. Keep the reconstituted suspension in a refrigerator (2-8°C) and discard after 7 days.
Overdose
Overdosage can lead to neurological sequelae including convulsions. Management is primarily symptomatic and supportive. Hemodialysis and peritoneal dialysis may be helpful in removing cefuroxime.
Pregnancy & Lactation
Pregnancy Category B. While animal studies have not shown harm, adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Cefuroxime is excreted in small amounts in breast milk; caution is advised when administered to a nursing mother.
