Description
Overview of the medicine
Roxim 200 mg capsule contains Cefuroxime Axetil, a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including respiratory tract, urinary tract, skin and soft tissue infections, and otitis media.
Uses & Indications
Dosage
Adults
For most infections, 250-500 mg twice daily. For uncomplicated urinary tract infections, 125-250 mg twice daily. For pharyngitis/tonsillitis, 250 mg twice daily for 5-10 days. For acute bacterial sinusitis, 250-500 mg twice daily for 10 days.
Elderly
Similar to adult dosage, but dose adjustment may be necessary in case of severe renal impairment.
Renal_impairment
Creatinine clearance 10-29 mL/min: 250 mg once daily. Creatinine clearance <10 mL/min: 125 mg once daily.
How to Take
Administer orally with food to enhance absorption. Do not crush or chew the capsule; swallow whole.
Mechanism of Action
Cefuroxime Axetil is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-3 hours after a meal.
Excretion
Primarily excreted unchanged in the urine by glomerular filtration and tubular secretion.
Half life
Approximately 1.2-1.5 hours in adults with normal renal function.
Absorption
Well absorbed from the gastrointestinal tract after oral administration, especially with food, and rapidly hydrolyzed to cefuroxime. Bioavailability is approximately 37-52%.
Metabolism
Cefuroxime Axetil is rapidly hydrolyzed in the intestinal mucosa and blood to release cefuroxime. Cefuroxime itself is not significantly metabolized.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, other cephalosporins, or any component of the formulation
- History of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems, monobactams)
Drug Interactions
Probenecid
Increases cefuroxime concentrations and prolongs its half-life.
Oral contraceptives
May reduce the efficacy of oral contraceptives (rarely significant, but advice on additional contraception might be given).
Antacids/H2-receptor antagonists/Proton pump inhibitors
May decrease the bioavailability of cefuroxime axetil by reducing gastric acidity.
Storage
Store at room temperature (below 30°C), away from moisture and direct light. Keep out of reach of children.
Overdose
Overdose symptoms may include cerebral irritation leading to convulsions. Management includes supportive care, gastric lavage if ingestion is recent, and hemodialysis/peritoneal dialysis may be considered to remove the drug.

Pregnancy Category B. Cefuroxime crosses the placenta. Use in pregnancy only if clearly needed. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but caution is advised.
