Description
Overview of the medicine
Sanfix (Cefixime) is a third-generation cephalosporin antibiotic used to treat various bacterial infections, including those of the respiratory tract, urinary tract, ear, throat, and gonorrhea.
Uses & Indications
Dosage
Adults
200-400 mg once daily or 200 mg every 12 hours, depending on the severity of infection. Duration usually 7-14 days.
Elderly
No specific dose adjustment is required unless there is severe renal impairment. Monitor renal function.
Renal_impairment
For creatinine clearance < 60 mL/min, dose adjustment is necessary. For CrCl < 20 mL/min, a dose of 200 mg once daily is recommended.
How to Take
Sanfix can be taken orally with or without food. For better gastrointestinal tolerance and absorption, especially the suspension form, it is often recommended with food.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Onset of action typically within 1 hour after administration.
Excretion
Primarily excreted unchanged in urine (about 50%) and bile.
Half life
Average plasma half-life of 3-4 hours.
Absorption
Approximately 40-50% orally absorbed, influenced by food. Peak plasma concentrations reached within 2-6 hours.
Metabolism
Minimal hepatic metabolism; no active metabolites identified.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime, other cephalosporins, or any component of the formulation.
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Warfarin
May increase prothrombin time and INR, leading to increased bleeding risk. Monitor coagulation parameters closely.
Probenecid
Increases Cefixime plasma concentrations by decreasing renal tubular secretion, potentially increasing side effects.
Oral Anticoagulants
Enhanced anticoagulant effect, necessitating dose adjustments and careful monitoring.
Storage
Store in a cool and dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Cefixime should be used during pregnancy only if clearly needed after careful assessment of risks and benefits. Excreted in breast milk in small amounts; use with caution in breastfeeding mothers.

Lab Monitoring
- For prolonged therapy, periodic assessment of renal, hepatic, and hematopoietic function is advisable. Monitor INR/PT in patients on concomitant warfarin.
Doctor Notes
- Emphasize the importance of completing the full course of therapy to prevent antibiotic resistance and ensure complete eradication of infection.
- Advise patients on potential gastrointestinal side effects and to seek medical attention if severe or persistent diarrhea, or signs of allergic reaction occur.
Patient Guidelines
- Take the medicine for the full prescribed duration, even if symptoms improve, to prevent the development of antibiotic resistance.
- Do not share this medicine with others; it is prescribed for your specific condition.
- Report any severe or persistent diarrhea, or signs of allergic reaction (e.g., rash, swelling) to your doctor immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Cefixime generally does not impair the ability to drive or operate machinery. However, if you experience dizziness or other central nervous system side effects, avoid such activities.
Lifestyle Advice
- Maintain good hygiene practices to prevent further infections. Stay hydrated by drinking plenty of fluids, especially if experiencing diarrhea. Avoid self-medication with antibiotics.
