Description
Overview of the medicine
Cefalexin is a first-generation cephalosporin antibiotic used to treat various bacterial infections. It works by interfering with the bacteria's cell wall formation, leading to bacterial cell death.
Uses & Indications
Dosage
Adults
250 mg to 500 mg every 6 hours, or 500 mg to 1 g every 12 hours. Maximum daily dose is 4 g.
Elderly
Dosage adjustment may be necessary in elderly patients with reduced renal function.
Renal_impairment
Dosage adjustment is necessary based on creatinine clearance. For CrCl 10-50 mL/min: 250 mg to 500 mg every 8-12 hours; for CrCl <10 mL/min: 250 mg to 500 mg every 12-24 hours.
How to Take
Orally, with or without food. To minimize gastrointestinal upset, it can be taken with food.
Mechanism of Action
Cefalexin is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to one or more of the penicillin-binding proteins (PBPs), thereby inhibiting the final transpeptidation step of peptidoglycan synthesis in the bacterial cell wall.
Pharmacokinetics
Onset
0.5 - 1 hour
Excretion
Primarily via renal excretion (glomerular filtration and tubular secretion); about 75-100% of the dose is excreted unchanged in the urine within 6-12 hours.
Half life
Approximately 1-1.2 hours in adults with normal renal function, prolonged in renal impairment.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Peak plasma concentrations are usually achieved within 1 hour after administration.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to cefalexin or any other cephalosporin antibiotics.
- •History of severe hypersensitivity reactions (e.g., anaphylaxis) to any beta-lactam antibiotic (e.g., penicillins).
Drug Interactions
Metformin
May increase metformin concentrations, leading to a risk of lactic acidosis.
Probenecid
May increase cefalexin levels by decreasing renal excretion.
Loop diuretics
May increase the risk of nephrotoxicity.
Oral anticoagulants
May potentiate the anticoagulant effect.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, epigastric distress, diarrhea, and hematuria. Management involves symptomatic and supportive treatment; gastric lavage or activated charcoal may be considered. Hemodialysis may be useful in severe cases.
Pregnancy & Lactation
FDA pregnancy category B. Use with caution in pregnancy only if clearly needed. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but caution is advised.
