Description
Overview of the medicine
Amoxicillin is a penicillin antibiotic used to treat a wide range of bacterial infections in infants and children. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Neonates
Lower doses, e.g., 20-30 mg/kg/day in divided doses, adjusted based on renal function.
Infants_children
Dosage is based on body weight and severity of infection. Typically 20-90 mg/kg/day in 2-3 divided doses. For 125mg/1.25ml drop, follow specific instructions from a physician or product leaflet regarding ml or drop count.
Renal_impairment
Dosage reduction is necessary in patients with severe renal impairment (creatinine clearance < 30 ml/min).
How to Take
Administer orally, with or without food. For pediatric drops, shake the bottle well before each use. Use the provided calibrated dropper for accurate dosing. Can be mixed with milk, formula, fruit juice, or water for infants.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion. About 60-70% of a dose is excreted in the urine within 6 hours.
Half life
Approximately 1-1.5 hours in adults with normal renal function; longer in neonates and patients with renal impairment.
Absorption
Rapidly and well absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Partially metabolized in the liver to inactive penicilloic acids.
Side Effects
Contraindications
- Hypersensitivity to amoxicillin, any other penicillin, or to any of the excipients.
- History of severe immediate hypersensitivity reaction to another beta-lactam agent (e.g., a cephalosporin, carbapenem or monobactam).
- Infectious mononucleosis (due to increased risk of maculopapular rash).
Drug Interactions
Probenecid
Decreases renal tubular secretion of amoxicillin, increasing blood levels and prolonging its half-life.
Allopurinol
Concomitant use with amoxicillin may increase the incidence of skin rashes.
Methotrexate
Reduced renal clearance of methotrexate, potentially leading to increased methotrexate toxicity.
Oral contraceptives
Amoxicillin may reduce the efficacy of oral contraceptives, leading to an increased risk of pregnancy (though clinical significance is debated).
Warfarin and other oral anticoagulants
Increased INR/bleeding risk. Close monitoring of coagulation parameters is recommended.
Storage
Store dry powder below 30°C in a cool, dry place, away from direct sunlight and moisture. Once reconstituted, store the suspension in a refrigerator (2-8°C) and discard any unused portion after the specified period. Do not freeze.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, crystalluria (leading to renal failure in some cases), and disturbances of fluid and electrolyte balance. Treatment is supportive; hemodialysis may be considered to remove amoxicillin from circulation.
Pregnancy & Lactation
Pregnancy Category B. Amoxicillin is considered generally safe for use during pregnancy. It is excreted in small amounts in breast milk; generally considered compatible with breastfeeding, but monitor for adverse effects in the infant (e.g., diarrhea, candidiasis, rash).

Lab Monitoring
- Periodic assessment of renal, hepatic, and hematopoietic function is advisable during prolonged therapy.
- Monitor complete blood count (CBC) and liver function tests (LFTs).
Doctor Notes
- Always consider local antibiotic resistance patterns when prescribing amoxicillin.
- Advise parents/caregivers on the importance of completing the full course of therapy and proper administration techniques for pediatric drops.
- Be alert for signs of hypersensitivity reactions, especially after the first dose.
Patient Guidelines
- Complete the entire course of medication as prescribed by the doctor, even if symptoms improve, to prevent recurrence and development of antibiotic resistance.
- Shake the bottle well before each use of the pediatric drop.
- Use the provided dropper to ensure accurate dosing.
- Report any severe or persistent side effects, especially severe rash or diarrhea, to your doctor immediately.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for the next scheduled dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Amoxicillin generally does not impair the ability to drive or operate machinery. However, if dizziness or allergic reactions occur, caution is advised.
Lifestyle Advice
- Maintain good hygiene to prevent the spread of infections.
- Ensure adequate fluid intake to prevent dehydration, especially if experiencing diarrhea.
