Description
Overview of the medicine
Simox 500 mg Capsule is an antibiotic belonging to the penicillin group. It is used to treat a wide variety of bacterial infections.
Uses & Indications
Dosage
Adults
Standard: 250-500 mg every 8 hours, or 875 mg every 12 hours depending on the severity and type of infection. Severe infections may require higher doses.
Elderly
Dosage should be adjusted based on renal function. Generally, no specific dose adjustment required for age alone if renal function is normal.
Renal_impairment
Dosage reduction or increased dosing interval is necessary. For creatinine clearance 10-30 mL/min: 250-500 mg every 12 hours. For <10 mL/min: 250-500 mg every 24 hours.
How to Take
Take orally, with or without food. Swallow the capsule whole with water. Complete the full prescribed course of treatment even if symptoms improve.
Mechanism of Action
Amoxicillin works by inhibiting the synthesis of bacterial cell walls. It binds to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Clinical effects usually within 30 minutes to 1 hour.
Excretion
Primarily excreted unchanged in the urine (50-70%) by renal tubular secretion and glomerular filtration within 6 hours.
Half life
Approximately 1-1.5 hours in adults with normal renal function. Prolonged in renal impairment.
Absorption
Rapidly and well absorbed from the gastrointestinal tract. Peak plasma concentrations are achieved within 1-2 hours after oral administration. Food does not significantly affect absorption.
Metabolism
About 10-25% is metabolized to inactive penicilloic acid.
Side Effects
Contraindications
- Known hypersensitivity to penicillin or any other beta-lactam antibiotics (e.g., cephalosporins).
- History of severe allergic reaction to amoxicillin or other penicillins.
- Infectious mononucleosis (due to increased risk of rash).
Drug Interactions
Probenecid
Increases and prolongs amoxicillin blood levels by reducing renal tubular secretion.
Allopurinol
Concurrent use significantly increases the incidence of skin rash, especially in hyperuricemic patients.
Tetracyclines
Bacteriostatic antibiotics like tetracyclines may antagonize the bactericidal effect of penicillin.
Oral Contraceptives
Amoxicillin may reduce the efficacy of oral contraceptives; advise additional non-hormonal contraception.
Anticoagulants (e.g., Warfarin)
May prolong prothrombin time; careful monitoring is required.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal symptoms (nausea, vomiting, diarrhea), fluid and electrolyte imbalance, and in severe cases, renal impairment or convulsions. Management is symptomatic and supportive, including hemodialysis if severe renal impairment occurs.
Pregnancy & Lactation
Pregnancy Category B. Amoxicillin is generally considered safe during pregnancy, but should only be used if clearly needed. It is excreted in small amounts in breast milk; usually not harmful to breastfed infants, but monitor for diarrhea or fungal infections in the infant. Consult your doctor before use.
