Description
Overview of the medicine
Simox 250 mg Capsule contains Amoxicillin, a broad-spectrum penicillin antibiotic used to treat a wide range of bacterial infections including those of the respiratory tract, urinary tract, skin, and soft tissues. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
250-500 mg every 8 hours, or 500-875 mg every 12 hours, depending on the severity and type of infection. For severe infections, up to 1g every 8 hours may be prescribed.
Elderly
No specific dose adjustment is required unless there is significant renal impairment. Monitor renal function.
Renal_impairment
Dosage adjustment is necessary for patients with creatinine clearance < 30 mL/min. E.g., for creatinine clearance 10-30 mL/min, 250-500 mg every 12 hours; for <10 mL/min, 250-500 mg every 24 hours.
How to Take
Take orally, with or without food. Complete the full course of treatment as prescribed by your doctor, even if symptoms improve. Swallow the capsule whole with water.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Within 30 minutes to 1 hour.
Excretion
Primarily excreted unchanged by the kidneys via tubular secretion and glomerular filtration.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, with bioavailability of about 70-90%. Peak plasma concentrations are reached in 1-2 hours.
Metabolism
Minimally metabolized; about 10-25% is metabolized to inactive penicilloic acid.
Side Effects
Contraindications
- Hypersensitivity to amoxicillin, other penicillins, or any component of the formulation.
- History of a severe hypersensitivity reaction (e.g., anaphylaxis) to another beta-lactam antibiotic (e.g., cephalosporins, carbapenems).
Drug Interactions
Warfarin
May prolong prothrombin time (INR) in patients receiving warfarin. Monitor INR closely.
Probenecid
Increases and prolongs amoxicillin blood levels by reducing renal tubular secretion.
Allopurinol
May increase the incidence of skin rash, especially in patients with hyperuricemia.
Tetracyclines
May interfere with the bactericidal effect of amoxicillin.
Oral Contraceptives
May reduce the efficacy of oral contraceptives (rarely significant, but advise backup contraception).
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastrointestinal symptoms like nausea, vomiting, and diarrhea may occur. Renal impairment can lead to amoxicillin crystalluria, potentially causing renal failure in severe cases. Treatment is supportive; hemodialysis can be used to remove amoxicillin from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk, but there are no adequate and well-controlled studies in pregnant women. Amoxicillin is generally considered safe during pregnancy when clearly needed. It is excreted in breast milk in small amounts; generally considered safe for nursing infants but monitor for potential diarrhea, candidiasis, or sensitization.

WHO Essential Medicine
YesClinical Trials
Amoxicillin has been extensively studied since its introduction in the early 1970s. Numerous clinical trials have established its efficacy and safety in treating a broad range of bacterial infections across various patient populations.
Lab Monitoring
- Renal function tests (for prolonged therapy or in patients with renal impairment)
- Liver function tests (rarely, if hepatotoxicity is suspected)
- Complete blood count (for prolonged therapy or if blood dyscrasias are suspected)
Doctor Notes
- Always emphasize the importance of completing the full course of therapy to patients to prevent antibiotic resistance and relapse.
- Carefully assess patients for any history of penicillin or cephalosporin allergy before prescribing.
- For patients with renal impairment, adjust the dosage appropriately to avoid accumulation and toxicity.
Patient Guidelines
- Take the medicine exactly as prescribed, for the full duration, even if you feel better.
- Do not share this medicine with others, even if they have similar symptoms.
- Inform your doctor if you experience any signs of an allergic reaction (rash, itching, swelling, severe dizziness, difficulty breathing).
- This antibiotic treats bacterial infections and is ineffective against viral infections like the common cold or flu.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
Amoxicillin is not known to impair the ability to drive or operate machinery. However, if side effects like dizziness or allergic reactions occur, avoid such activities.
Lifestyle Advice
- Maintain good personal hygiene to prevent the spread of infection.
- Drink plenty of fluids to stay hydrated, especially if experiencing diarrhea.
- Avoid self-medication with antibiotics and always complete the prescribed course.
