Description
Overview of the medicine
Sompraz 20 mg capsule contains Esomeprazole, a proton pump inhibitor that works by decreasing the amount of acid your stomach makes. It is used to treat acid-related diseases of the stomach and gut.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For healing of erosive esophagitis: 20-40 mg once daily for 4 to 8 weeks. For H. pylori eradication: 20 mg twice daily for 7-14 days with appropriate antibiotics. For Zollinger-Ellison Syndrome: Initial dose of 40 mg twice daily, adjusted individually.
Elderly
No dose adjustment is generally required for elderly patients, but caution is advised in those with severe hepatic impairment.
Renal_impairment
No dose adjustment is required in patients with renal impairment.
How to Take
Take the capsule orally, swallowed whole with water. Do not chew or crush the capsule. It should be taken at least one hour before a meal.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the gastric H+/K+-ATPase (proton pump) in the parietal cells of the stomach. This action blocks the final step of acid production, leading to a reduction in gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Primarily via urine (approximately 80%) and feces (approximately 20%) as inactive metabolites.
Half life
Approximately 1-1.5 hours
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 50-60% for a 20 mg dose, increasing with repeated dosing.
Metabolism
Extensively metabolized in the liver via CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time; monitor patients for increased bleeding risk.
Tacrolimus
Increased serum concentrations of tacrolimus; monitor tacrolimus concentrations and renal function.
Clopidogrel
Potential for reduced antiplatelet effect of clopidogrel; consider alternative PPIs or antiplatelet therapy.
Methotrexate
Increased methotrexate levels, particularly with high-dose methotrexate; monitor for toxicity.
Nelfinavir, Atazanavir
Decreased plasma levels of nelfinavir and atazanavir; avoid concomitant use.
Diazepam, Citalopram, Imipramine, Clomipramine, Phenytoin
Esomeprazole may increase plasma concentrations of drugs metabolized by CYP2C19.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is limited experience with Esomeprazole overdose. In case of an overdose, symptomatic and supportive measures should be taken. Esomeprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Esomeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.

Lab Monitoring
- Monitor magnesium levels, especially in patients on prolonged therapy (three months or more) or those taking other drugs that may lower magnesium levels.
- Periodic monitoring of liver function tests may be considered in patients with hepatic impairment.
Doctor Notes
- Before initiating Esomeprazole therapy for gastric ulcer, rule out the presence of malignancy.
- Monitor patients for hypomagnesemia, especially those on prolonged therapy or concomitant diuretics.
- Consider the risk of C. difficile infection with prolonged use.

- Take the medicine exactly as prescribed by your doctor.
- Do not stop taking Esomeprazole without consulting your doctor, even if your symptoms improve.
- Report any new or worsening symptoms to your doctor.
