Overview of the medicine
Sompraz-40 mg capsule is a proton pump inhibitor used to treat conditions involving excessive stomach acid, such as GERD, peptic ulcers, and Zollinger-Ellison syndrome. It works by reducing the amount of acid produced in the stomach.
Uses & Indications
Dosage
Adults
For GERD, 20 mg or 40 mg once daily for 4 to 8 weeks. For Zollinger-Ellison Syndrome, initial dose 40 mg twice daily, adjusted as needed. For H. pylori eradication, 40 mg once daily with amoxicillin and clarithromycin for 10 days.
Elderly
No specific dose adjustment is needed in the elderly.
Renal_impairment
No dose adjustment is generally necessary for patients with renal impairment.
How to Take
Sompraz-40 mg capsule should be taken whole with water at least one hour before a meal. Do not chew or crush the capsule.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically and irreversibly blocks the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This inhibition prevents the final step of acid production, thereby reducing gastric acid secretion.

Absorption
Rapidly absorbed after oral administration; peak plasma concentrations achieved within 1-2 hours. Bioavailability is approximately 64% after a single 40 mg dose, increasing to 89% after repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased absorption of digoxin due to increased gastric pH.
Diazepam
Reduced clearance of diazepam (CYP2C19 substrate).
Warfarin
Increased INR and prothrombin time have been reported, requiring monitoring.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Methotrexate
Increased and prolonged serum levels of methotrexate and/or its metabolite.
Atazanavir, Nelfinavir
Significant reduction in plasma levels of these antiretrovirals.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Limited experience with overdose. Symptoms may include lethargy, drowsiness, blurred vision, tachycardia, nausea, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Excretion into breast milk is unknown, caution is advised.
