Description
Overview of the medicine
Sompraz 40 mg Tablet contains Esomeprazole, a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions like acid reflux, heartburn, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD with Erosive Esophagitis: 20 mg or 40 mg once daily for 4 to 8 weeks. Maintenance: 20 mg once daily. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted as needed.
Elderly
No dose adjustment generally required, but consider lower starting dose due to potential for reduced hepatic function.
Renal_impairment
No dose adjustment necessary for patients with renal impairment.
How to Take
Take the tablet whole with a glass of water, at least one hour before a meal. Do not crush or chew. If unable to swallow, dissolve in non-carbonated water and drink immediately.
Mechanism of Action
Esomeprazole selectively inhibits the gastric parietal cell H+/K+-ATPase (proton pump), thereby blocking the final step of acid secretion. This leads to a dose-dependent inhibition of both basal and stimulated gastric acid secretion.

Excreted primarily via the urine (approximately 80%) and feces (approximately 20%) as metabolites.

Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached in 1-2 hours. Bioavailability is about 64% after a single 40 mg dose.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Esomeprazole, other substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir (an antiretroviral drug).

Digoxin
Increased digoxin levels (monitor closely).
Warfarin
Increased INR/prothrombin time (monitor closely).
Tacrolimus
Increased tacrolimus levels (monitor concentrations).
Clopidogrel
Reduced antiplatelet effect of clopidogrel (avoid concomitant use).
Methotrexate
Increased methotrexate levels (monitor closely).
