Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), stomach ulcers, and other conditions involving excessive stomach acid.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. For Zollinger-Ellison Syndrome: Initial dose 40 mg twice daily, adjusted as needed. For H. pylori eradication: 20 mg twice daily for 7-14 days in combination with antibiotics.
Elderly
No specific dosage adjustment is usually necessary for the elderly, but caution should be exercised in severe hepatic impairment.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Take one tablet whole, with a glass of water, at least 30-60 minutes before a meal, preferably in the morning. Do not chew or crush the tablet.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase enzyme (the proton pump) in the parietal cells of the stomach. This irreversible inhibition blocks the final step in acid production, leading to a sustained reduction in gastric acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour; maximal acid inhibition within 2-4 hours.
Excretion
Approximately 80% of the dose is excreted as metabolites in the urine, and the remainder (about 20%) in the feces.
Half life
Plasma elimination half-life is approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is about 64% after a single dose, increasing to about 89% after repeated once-daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and to a lesser extent by CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased absorption of digoxin due to increased gastric pH.
Warfarin
Increased INR and prothrombin time, increasing the risk of bleeding.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Increased plasma concentrations and prolonged elimination of methotrexate, potentially leading to toxicity.
Atazanavir/Nelfinavir
Reduced plasma levels of these antiretrovirals, leading to loss of therapeutic effect.
