Overview of the medicine
Sonexa 4 mg Tablet contains Bromazepam, which is a benzodiazepine used to treat anxiety, tension, and insomnia (difficulty sleeping) for a short period.
Uses & Indications
Dosage
Adults
For anxiety: 3-6 mg daily in divided doses. In more severe cases: 6-18 mg daily in divided doses. Max 30 mg/day.
Elderly
Start with lower doses, e.g., 1.5-3 mg daily in divided doses, adjusted according to response.
Renal_impairment
Dose reduction may be necessary due to reduced excretion of metabolites. Use with caution.
How to Take
Take orally with or without food. Swallow the tablet whole with water.
Mechanism of Action
Bromazepam enhances the activity of gamma-aminobutyric acid (GABA), a neurotransmitter in the brain, which helps to calm the nervous system and reduce anxiety.

Within 20-60 minutes.
Excretion
Mainly excreted in urine as metabolites.
Half life
10-20 hours (intermediate-acting).
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1-2 hours.

Primarily metabolized in the liver via CYP450 enzymes (mainly CYP3A4) to active and inactive metabolites.

Contraindications

Drug Interactions
CYP3A4 inducers (e.g., rifampicin, carbamazepine)
May decrease bromazepam plasma levels.
CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, cimetidine)
May increase bromazepam plasma levels.
CNS depressants (alcohol, opioids, other anxiolytics, antidepressants)
Increased sedative effect, respiratory depression.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, confusion, lethargy, ataxia, hypotension, and respiratory depression. Management is supportive; flumazenil, a benzodiazepine antagonist, can be used in severe cases.
Pregnancy & Lactation
Pregnancy Category D. Not recommended during pregnancy, especially during the first trimester and before delivery. Passes into breast milk and is not recommended during breastfeeding.
