Description
Overview of the medicine
Sultolin 100 mcg Inhaler contains Salbutamol, a short-acting beta-2 adrenergic agonist used for the rapid relief of bronchospasm in conditions like asthma and COPD. It relaxes the smooth muscles of the airways, making breathing easier.
Uses & Indications
Dosage
Adults
For acute asthma symptoms, 1 or 2 inhalations (100 or 200 mcg) as needed. For prevention of exercise-induced bronchospasm, 2 inhalations (200 mcg) 10-15 minutes before exercise.
Elderly
Same as adult dosage, but caution with cardiovascular disease.
Renal_impairment
No dosage adjustment typically required, but monitor for adverse effects.
How to Take
Inhale deeply and slowly through the mouth while pressing down on the canister. Hold breath for 10 seconds. Shake the inhaler well before each use. A spacer device may be recommended, especially for children or those with difficulty coordinating inhalation.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors primarily in the bronchial smooth muscles. This stimulation leads to the activation of adenyl cyclase, increasing intracellular cyclic AMP (cAMP) levels. Elevated cAMP levels result in relaxation of bronchial smooth muscle and inhibition of mediator release from mast cells, thus causing bronchodilation.
Pharmacokinetics
Onset
Within 5-15 minutes
Excretion
Mainly excreted in urine as unchanged drug and its metabolite within 72 hours.
Half life
3.8 to 5 hours
Absorption
Approximately 10-20% of the inhaled dose reaches the lungs, while the remainder is swallowed and absorbed from the gastrointestinal tract. Peak plasma levels occur within 2-3 hours after inhalation.
Metabolism
Metabolized primarily in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- Hypersensitivity to salbutamol or any component of the inhaler
- Threatened abortion during the first or second trimester of pregnancy
Drug Interactions
Digoxin
May decrease serum digoxin levels.
Diuretics
May potentiate hypokalemia.
Beta-blockers
May block the bronchodilatory effect of salbutamol and cause severe bronchospasm.
MAOIs/Tricyclic Antidepressants
May potentiate the cardiovascular effects of salbutamol.
Storage
Store below 30°C. Protect from direct sunlight and frost. Do not freeze. Keep out of reach of children.
Overdose
Symptoms include tachycardia, tremors, palpitations, headache, and restlessness. Severe hypokalemia and metabolic acidosis may occur. Treatment is symptomatic and supportive. Consider cardioselective beta-blockers with caution.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit justifies the potential risk to the fetus (Category C). Salbutamol is excreted in breast milk; caution advised during lactation.

- Emphasize proper inhaler technique.
- Advise patients that this is a 'reliever' medication, not a 'controller'.
- Assess for overuse, which may indicate poor asthma control.
Patient Guidelines
- Do not exceed the prescribed dose.
- Seek medical attention if symptoms worsen or if the usual dose becomes less effective.
- Rinse mouth with water after use to prevent oral candidiasis.
- Keep the inhaler clean.
Missed Dose Advice
