Description
Overview of the medicine
Sultolin 5 mg Respirator Solution contains Salbutamol, a fast-acting bronchodilator used to relieve symptoms of asthma and COPD.
Uses & Indications
Dosage
Adults
The usual starting dose is 2.5 mg to 5 mg nebulized 3-4 times daily. It can be diluted with sterile normal saline to a total volume of 2.5 to 5 mL.
Elderly
No specific dose adjustment is required, but caution is advised.
Renal_impairment
No specific dose adjustment is generally required, but close monitoring is recommended.
How to Take
Administer by nebulization using an appropriate nebulizer device. Do not inject or take orally. Ensure proper dilution with sterile normal saline as advised by a healthcare professional.
Mechanism of Action
Salbutamol acts as a selective beta-2 adrenoceptor agonist on the smooth muscle of the airways, leading to relaxation of bronchial smooth muscle and bronchodilation. This effect is mediated by activation of adenyl cyclase, increasing intracellular cyclic AMP (cAMP) levels.
Pharmacokinetics
Onset
5-15 minutes
Excretion
Primarily excreted in the urine as unchanged drug and its metabolite.
Half life
3.8 to 5 hours (oral), 2.7 to 5 hours (inhalation)
Absorption
Rapidly absorbed from the respiratory tract after inhalation. Systemic absorption of the nebulized solution is typically low but varies.
Metabolism
Undergoes extensive metabolism in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- Hypersensitivity to salbutamol or any component of the solution
- Thyrotoxicosis (relative contraindication)
Drug Interactions
Diuretics
May cause ECG changes and hypokalemia. Concurrent use requires caution.
Beta-blockers
May antagonize the effect of salbutamol and can cause severe bronchospasm in asthmatic patients. Non-selective beta-blockers should generally be avoided.
MAOIs and Tricyclic Antidepressants
May potentiate the cardiovascular effects of salbutamol. Use with extreme caution.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include tremor, headache, tachycardia, palpitations, arrhythmia, and hypokalemia. Treatment is symptomatic and supportive, mainly involving cessation of treatment and careful monitoring of cardiac status and serum potassium levels.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether salbutamol is excreted in breast milk; use with caution in nursing mothers.

Lab Monitoring
- Serum potassium levels (especially with high doses or concomitant use of diuretics)
- ECG monitoring (in patients with pre-existing cardiac conditions or during overdose)
Doctor Notes
- Emphasize proper inhaler technique for optimal drug delivery.
- Monitor patients for signs of hypokalemia, especially with concomitant diuretic use or severe asthma exacerbations.
- Advise patients that this is a 'reliever' medication and not for daily maintenance unless specifically prescribed.
Patient Guidelines
- Always use your nebulizer device correctly as instructed by your doctor or pharmacist.
- Do not exceed the prescribed dose.
- Seek immediate medical attention if your breathing worsens or if you need to use your inhaler more often than usual.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
May cause tremor or dizziness in some patients. Patients should be advised to exercise caution when driving or operating machinery until they know how Sultolin affects them.
Lifestyle Advice
- Avoid smoking, as it can worsen respiratory conditions.
- Maintain a healthy lifestyle with regular exercise and a balanced diet.
- Avoid known triggers for asthma or COPD exacerbations.
