Description
Overview of the medicine
Sultolin 200 mcg Inhalation Capsule contains Salbutamol, a fast-acting bronchodilator used to relieve symptoms of asthma and COPD.
Uses & Indications
Dosage
Adults
200-400 mcg (1-2 capsules) via inhaler 3-4 times daily as needed. For prevention of exercise-induced bronchospasm: 200 mcg 10-15 minutes before exercise.
Elderly
Similar to adult dose, but may require lower initial doses. Monitor closely.
Renal_impairment
No specific dose adjustment generally needed, but monitor closely for side effects.
How to Take
For oral inhalation only. The capsule should be placed into a suitable dry powder inhaler device (e.g., Rotahaler) and the contents inhaled as directed by the physician. Do not swallow the capsule.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors in bronchial smooth muscle, causing relaxation and bronchodilation. It also inhibits the release of mast cell mediators.
Pharmacokinetics
Onset
5-15 minutes
Excretion
Excreted mainly in the urine, both as unchanged drug and its metabolite.
Half life
4-6 hours (systemic)
Absorption
Rapidly absorbed after inhalation. Systemic bioavailability is low due to first-pass metabolism.
Metabolism
Primarily metabolized in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- Hypersensitivity to salbutamol or any component of the formulation
- Tachyarrhythmia
Drug Interactions
Diuretics
May cause ECG changes and/or hypokalemia, especially with high doses.
Beta-blockers
Can cause severe bronchospasm in asthmatics and inhibit the bronchodilatory effect of salbutamol.
Tricyclic Antidepressants (TCAs) & MAOIs
May potentiate the cardiovascular effects of salbutamol.
Storage
Store below 30°C in a dry place, protected from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms include tachycardia, tremors, palpitations, headache, and hypokalemia. Management involves symptomatic and supportive care, monitoring electrolyte levels.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Small amounts may be excreted in breast milk; use with caution.

Lab Monitoring
- Monitor serum potassium levels (especially with high doses or concurrent use of diuretics)
- Monitor blood glucose levels in diabetic patients
Doctor Notes
- Educate patients on proper inhaler technique for optimal drug delivery.
- Advise patients against overuse; frequent use (more than 2-3 times weekly for symptom control) should prompt reassessment of asthma/COPD management.
- Be mindful of potential drug interactions, especially with non-selective beta-blockers and certain diuretics.
Patient Guidelines
- Always use an appropriate inhaler device as directed by your physician.
- Do not swallow the capsule; it is for inhalation only.
- Rinse your mouth with water after each use to reduce the risk of oral candidiasis.
- Seek medical attention immediately if your symptoms worsen or do not improve.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. Do not double dose.
Driving Precautions
Generally, salbutamol does not affect the ability to drive or operate machinery. However, if side effects like dizziness, tremor, or blurred vision occur, avoid driving until these subside.
Lifestyle Advice
- Avoid known triggers that worsen asthma/COPD symptoms (e.g., smoke, allergens).
- Maintain a healthy lifestyle, including regular exercise if advised by your physician.
- Quit smoking if applicable.
