Overview of the medicine
Cefixime is a broad-spectrum cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
200 mg to 400 mg daily, as a single dose or in two divided doses, for 7-14 days depending on the infection.
Elderly
No dosage adjustment needed unless severe renal impairment is present.
Renal_impairment
Dosage adjustment is necessary in patients with creatinine clearance < 60 mL/min. For CrCl 21-60 mL/min, 75% of the standard dose; for CrCl < 20 mL/min, 50% of the standard dose.
How to Take
Can be taken with or without food. However, absorption is slightly increased when taken with food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis, leading to bacterial lysis and death. It has a bactericidal effect.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in urine (about 50%) and bile.
Half life
Average plasma half-life is 3-4 hours.
Absorption
Approximately 40-50% orally absorbed, influenced by food.
Metabolism
Not extensively metabolized; some active metabolites may be formed.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •History of severe hypersensitivity reactions to penicillin.
Drug Interactions
Probenecid
Increases cefixime plasma levels and prolongs its half-life by reducing renal excretion.
Carbamazepine
Concomitant use may increase carbamazepine levels.
Warfarin and other anticoagulants
May increase prothrombin time (INR), requiring careful monitoring and dose adjustment.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be supportive.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed. Caution when administered to a nursing woman.
