Cefixime is a broad-spectrum third-generation cephalosporin antibiotic used to treat a variety of bacterial infections in infants and children. It is effective against both Gram-positive and Gram-negative bacteria.

Adults

Elderly
Not applicable for pediatric drop formulation.
Pediatric
For infants and children (6 months to 12 years): 8 mg/kg/day administered as a single dose or in two divided doses every 12 hours. For children over 12 years or weighing more than 50 kg, the adult dose may be considered. Duration of treatment varies by infection, typically 7-14 days.
Renal_impairment
Dosage adjustment is necessary for severe renal impairment (creatinine clearance <30 mL/min). Consult a physician for specific guidance.
How to Take
T-Cef Pediatric Drop is for oral administration. Shake the bottle well before each use. Administer the prescribed dose using the provided dropper, directly into the child's mouth or mixed with a small amount of water, milk, or infant formula. Can be taken with or without food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby disrupting the formation of the peptidoglycan layer, which is essential for the integrity and survival of the bacterial cell. This leads to bacterial cell lysis and death.

Peak plasma concentrations are achieved 2-4 hours after administration.
Excretion
Primarily excreted unchanged in the urine (about 50-60% of the absorbed dose) and also through bile/feces.
Half life
Plasma half-life is typically 3-4 hours, potentially longer in patients with renal impairment.
Absorption
Approximately 40-50% orally absorbed. Food can slightly delay absorption but does not significantly alter the extent.
Metabolism
Cefixime undergoes minimal hepatic metabolism; approximately 10% is metabolized.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •History of hypersensitivity to penicillin (due to potential cross-reactivity).
Drug Interactions
Nifedipine
May increase cefixime peak plasma levels and AUC.
Carbamazepine
May increase carbamazepine plasma levels. Monitoring is recommended.
Warfarin and other anticoagulants
May increase prothrombin time (INR) and risk of bleeding. Close monitoring is advised.
Storage
Store the dry powder at room temperature, below 30°C, and protect from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard any unused portion after the specified shelf life.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment is primarily symptomatic and supportive.
Pregnancy & Lactation
Cefixime is classified as Pregnancy Category B. While pediatric drops are not typically for pregnant or lactating women, for general Cefixime, it should be used during pregnancy only if clearly needed. Small amounts are excreted in breast milk; caution should be exercised when administered to a nursing mother.
