Description
Overview of the medicine
Telfex (Fexofenadine) is a non-sedating antihistamine used to relieve allergy symptoms such as sneezing, runny nose, itchy watery eyes, and itchy throat, as well as symptoms of chronic idiopathic urticaria like itching and hives.
Uses & Indications
Dosage
Adults
Seasonal allergic rhinitis: 60 mg twice daily or 180 mg once daily. Chronic idiopathic urticaria: 180 mg once daily.
Elderly
No dosage adjustment generally required, but caution with renal impairment.
Renal_impairment
For severe renal impairment (CrCl <10 mL/min), initial dose of 60 mg once daily is recommended.
How to Take
Oral administration. Can be taken with or without food. Avoid taking with fruit juice (grapefruit, orange, apple) as it may reduce absorption.
Mechanism of Action
Fexofenadine is a selective peripheral H1-receptor antagonist. It competitively binds to H1 receptors, preventing histamine from binding and exerting its effects, thereby reducing allergy symptoms without significantly crossing the blood-brain barrier, which minimizes sedation.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Mainly excreted unchanged in bile (approximately 80%) and urine (approximately 11%).
Half life
Approximately 14 hours
Absorption
Rapidly absorbed after oral administration; Tmax is approximately 1-3 hours.
Metabolism
Minimally metabolized (less than 5%) in the liver.
Side Effects
Contraindications
- Known hypersensitivity to fexofenadine or any component of the formulation.
Drug Interactions
Erythromycin or Ketoconazole
May increase fexofenadine plasma concentrations, though clinical significance is usually minimal.
Antacids (containing aluminum and magnesium)
May reduce fexofenadine absorption. Separate administration by at least 2 hours.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose management: Symptoms of overdose may include dizziness, drowsiness, and dry mouth. Management is symptomatic and supportive. Hemodialysis does not effectively remove fexofenadine from blood.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk, caution advised when administered to a nursing mother.
