Description
Overview of the medicine
Traxon 1 gm Injection contains Ceftriaxone, a third-generation cephalosporin antibiotic used to treat various bacterial infections including severe sepsis, meningitis, pneumonia, and urinary tract infections. It is administered parenterally.
Uses & Indications
Dosage
Adults
Typical dose is 1-2 gm once daily (OD) intravenously (IV) or intramuscularly (IM), depending on infection severity. Max 4 gm/day.
Elderly
No specific dose adjustment typically required unless severe renal/hepatic impairment.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. For severe impairment (creatinine clearance <10 ml/min), dose should not exceed 2 gm daily without close monitoring.
How to Take
For intravenous (IV) administration, reconstitute with sterile water for injection and infuse over 30 minutes. For intramuscular (IM) administration, reconstitute with lidocaine 1% solution and inject deeply into a large muscle mass.
Mechanism of Action
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It is bactericidal.
Pharmacokinetics
Onset
Peak plasma concentrations within 2-3 hours after IM injection, or rapidly after IV infusion.
Excretion
Excreted via both renal (kidneys) and biliary (bile) routes. No dosage adjustment needed for mild to moderate renal or hepatic impairment alone.
Half life
Approximately 6-9 hours (prolonged in neonates and renal/hepatic impairment).
Absorption
Rapid and complete absorption after intramuscular (IM) administration; high bioavailability.
Metabolism
Partially metabolized in the liver, primarily to inactive metabolites. About 33-67% excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to ceftriaxone or any other cephalosporin antibiotics
- History of severe hypersensitivity reaction to any beta-lactam antibiotic (e.g., penicillin)
- Neonates with hyperbilirubinemia, especially premature neonates (due to risk of bilirubin encephalopathy)
Drug Interactions
Probenecid
May decrease ceftriaxone renal clearance, increasing its plasma concentration (though not clinically significant for ceftriaxone).
Aminoglycosides
Synergistic effect against some bacteria, but may increase nephrotoxicity risk (though less common with ceftriaxone).
Calcium-containing solutions
Concomitant administration is contraindicated in neonates due to risk of precipitation; avoid in all patients within 48 hours of ceftriaxone.
Oral anticoagulants (e.g., Warfarin)
Increased INR/bleeding risk. Monitor coagulation parameters closely.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Reconstituted solution should be used immediately or within specified periods (e.g., 6 hours at room temperature or 24 hours at 2-8°C).
Overdose
In case of overdose, symptoms may include nausea, vomiting, diarrhea, and central nervous system effects. Treatment is symptomatic and supportive. Ceftriaxone is not significantly removed by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Should be used during pregnancy only if clearly needed. Excreted in breast milk in small amounts; use with caution in breastfeeding mothers.

Doctor Notes
- Consider patient's allergy history carefully before administration.
- Monitor renal and hepatic function, especially during prolonged therapy.
- Avoid co-administration with calcium-containing solutions.
- Educate patients on completing the full course of treatment.
Patient Guidelines
- Complete the full course of antibiotics even if symptoms improve.
- Inform your doctor about any history of allergies, especially to penicillin or cephalosporins.
- Report any unusual side effects, especially severe diarrhea, rash, or breathing difficulties.
- Do not self-medicate or share this medication with others.
Missed Dose Advice
If a dose is missed, administer it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
