Description
Overview of the medicine
Traxon 2 gm Injection contains Ceftriaxone, a broad-spectrum third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Typically 1-2 gm once daily intravenously or intramuscularly, or 0.5-1 gm every 12 hours for severe infections; up to 4 gm/day.
Elderly
No specific dosage adjustment usually required unless severe renal or hepatic impairment.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. For severe impairment (CrCl < 10 mL/min), maximum dose should not exceed 2 gm daily.
How to Take
For intravenous (IV) administration, reconstitute with sterile water for injection and infuse slowly over 30 minutes. For intramuscular (IM) administration, reconstitute with lidocaine 1% (without epinephrine) and inject deep into a large muscle.
Mechanism of Action
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, within minutes after IV administration.
Excretion
Approximately 50-60% excreted unchanged in urine and 40-50% in bile/feces.
Half life
Approximately 5.8 to 8.7 hours (prolonged in neonates and elderly).
Absorption
Rapid and complete absorption after intramuscular administration; IV administration provides immediate bioavailability.
Metabolism
Limited hepatic metabolism; primarily metabolized to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to ceftriaxone, any cephalosporins, or penicillin
- Neonates with hyperbilirubinemia, especially premature neonates
- Concomitant use with IV calcium-containing solutions in neonates
Drug Interactions
Loop diuretics
Increased risk of nephrotoxicity with high doses of ceftriaxone.
Aminoglycosides
Synergistic effect against some organisms, but administer separately to avoid precipitation.
Oral anticoagulants
May increase the anticoagulant effect. Monitor INR/PT.
Calcium-containing solutions
Contraindicated in neonates; in older patients, flush lines between administration.
Storage
Store below 30°C. Protect from light. Reconstituted solution should be used immediately or within 6 hours if refrigerated.
Overdose
Overdose symptoms may include nausea, vomiting, and diarrhea. In severe cases, neurological symptoms (e.g., seizures) can occur. Treatment is symptomatic and supportive. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.
