Description
Overview of the medicine
Trialon 40 mg Injection contains Triamcinolone Acetonide, a synthetic corticosteroid. It is used to treat various inflammatory and autoimmune conditions by suppressing the immune system and reducing inflammation.
Uses & Indications
Dosage
Adults
Intramuscular injection: 40-80 mg once every 1-6 weeks, depending on the condition and patient response. Intra-articular/Intrabursal: 5-40 mg.
Elderly
Lower doses may be considered; caution with co-morbidities.
Renal_impairment
No specific dosage adjustment required, but caution is advised.
How to Take
For intramuscular, intra-articular, intrabursal, or intralesional injection only. Not for intravenous use. Administer deeply into the gluteal muscle for IM use. Shake well before use.
Mechanism of Action
Triamcinolone Acetonide binds to glucocorticoid receptors, modifying gene expression and leading to multiple anti-inflammatory and immunosuppressive effects. It inhibits prostaglandin and leukotriene synthesis, reduces capillary permeability, and stabilizes lysosomal membranes.
Pharmacokinetics
Onset
Within hours, full effect may take days for depot injection.
Excretion
Excreted mainly in urine, with some fecal excretion.
Half life
Biological half-life is approximately 18-36 hours; plasma half-life is shorter.
Absorption
Well absorbed after intramuscular injection, providing sustained release. Peak plasma levels usually occur within 8-24 hours.
Metabolism
Primarily metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Systemic fungal infections
- Hypersensitivity to Triamcinolone or any component
- Live or live-attenuated vaccines during high-dose corticosteroid therapy
Drug Interactions
NSAIDs
Increased risk of gastrointestinal ulceration.
Warfarin
Effects may be altered; monitor INR.
Diuretics (thiazide, loop)
May enhance potassium depletion.
Insulin/Oral hypoglycemics
May increase blood glucose, requiring dose adjustments.
Phenytoin, Carbamazepine, Rifampin, Barbiturates
May increase corticosteroid metabolism, reducing its effect.
Storage
Store at controlled room temperature (20°C to 25°C). Protect from freezing. Do not refrigerate. Keep out of reach of children.
Overdose
Acute overdose is rare. Chronic overdose can lead to Cushing's syndrome, adrenal suppression. Management includes symptomatic and supportive care, gradual dose reduction.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Corticosteroids are excreted in breast milk; caution advised, weighing benefits vs. risks.
