Description
Overview of the medicine
Triphin 1 gm Injection contains Triphincillin Sodium, a broad-spectrum beta-lactam antibiotic effective against a wide range of gram-positive and gram-negative bacteria. It is used to treat various severe bacterial infections.
Uses & Indications
Dosage
Adults
For severe infections, 1 gm IV/IM every 8-12 hours. Dosage may vary based on infection severity and pathogen susceptibility. Max dose 4 gm/day.
Elderly
Similar to adult dosage, but caution should be exercised and dose adjustment may be necessary based on renal function.
Renal_impairment
Dose reduction required. For creatinine clearance 30-50 mL/min: 1 gm every 12-24 hours; <30 mL/min: 1 gm every 24-48 hours. Hemodialysis patients require a supplementary dose after dialysis.
How to Take
Triphin 1 gm Injection can be administered via slow intravenous (IV) infusion (over 30-60 minutes) or deep intramuscular (IM) injection. For IM administration, a suitable diluent (e.g., lidocaine solution) may be used to reduce pain.
Mechanism of Action
Triphincillin acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, which prevents the cross-linking of peptidoglycan polymers, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Rapid, typically within minutes after intravenous administration.
Excretion
Mainly excreted by the kidneys (glomerular filtration and tubular secretion) into the urine. A small portion may be excreted in bile.
Half life
Approximately 1-2 hours in adults with normal renal function.
Absorption
Rapid and complete absorption after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are achieved shortly after IV infusion.
Metabolism
Minimal hepatic metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to Triphincillin, other beta-lactam antibiotics (e.g., penicillins, cephalosporins), or any component of the formulation.
- Patients with a history of severe immediate hypersensitivity reactions to any beta-lactam antibiotic.
Drug Interactions
Probenecid
May increase and prolong serum concentrations of Triphincillin by decreasing renal tubular secretion.
Aminoglycosides
Physical incompatibility; do not mix in the same syringe or infusion bottle. Administer separately.
Oral Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect, requiring closer monitoring of INR and dose adjustment.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children. Reconstituted solution should be used immediately or stored as per specific instructions (e.g., refrigerated for up to 24 hours).
Overdose
In case of overdose, supportive and symptomatic treatment should be initiated. Hemodialysis may be useful in enhancing the elimination of Triphincillin from the body, especially in patients with impaired renal function.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed. Triphincillin is excreted in breast milk; caution should be exercised when administered to a nursing mother. Consult your doctor.

Lab Monitoring
- Renal function tests (e.g., serum creatinine, BUN) especially in patients with pre-existing renal impairment or during prolonged therapy.
- Liver function tests (e.g., ALT, AST) if treatment is prolonged or in patients with hepatic dysfunction.
- Complete blood count (CBC) during extended treatment courses.
Doctor Notes
- Assess patient for a history of hypersensitivity reactions to beta-lactam antibiotics prior to administration.
- Monitor renal function, especially in elderly patients or those with pre-existing renal impairment, and adjust dosage accordingly.
- Be aware of potential drug interactions, particularly with probenecid and oral anticoagulants.
Patient Guidelines
- Complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
- Report any severe side effects or allergic reactions immediately to your healthcare provider.
- Do not share this medicine with others, and do not use it for conditions for which it was not prescribed.
Missed Dose Advice
If a dose is missed, administer it as soon as remembered. However, if it is almost time for the next scheduled dose, skip the missed dose and continue with the regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Triphin may cause dizziness or lightheadedness in some patients. Patients should be advised to exercise caution when driving or operating machinery until they know how the medication affects them.
Lifestyle Advice
- Maintain adequate hydration by drinking plenty of fluids during the course of treatment.
- Avoid alcohol consumption during antibiotic treatment, as it may worsen side effects or reduce efficacy.
