Description
Overview of the medicine
Triphin 500 mg Injection is a broad-spectrum antibiotic belonging to the cephalosporin class. It is used to treat a wide range of bacterial infections in various parts of the body.
Uses & Indications
Dosage
Adults
500 mg - 1 g intravenously (IV) or intramuscularly (IM) every 12-24 hours, depending on the severity of infection.
Elderly
No specific dose adjustment is typically required, but caution is advised in patients with significant renal impairment.
Renal_impairment
Dose adjustment may be necessary in severe renal impairment (CrCl < 30 mL/min). Consult physician for specific guidelines.
How to Take
Administer slowly by intravenous (IV) infusion over 30-60 minutes or by deep intramuscular (IM) injection. Reconstitute the powder with appropriate solvent (e.g., sterile water for injection, lidocaine solution for IM).
Mechanism of Action
Triphin acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, which are essential for peptidoglycan synthesis, thereby leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Within 30 minutes to 2 hours depending on administration route.
Excretion
Excreted mainly unchanged via the kidneys (glomerular filtration and tubular secretion) and also partially via biliary excretion.
Half life
Approximately 6-9 hours.
Absorption
Rapid and complete absorption after intramuscular (IM) administration, achieving peak plasma concentrations within 2-3 hours. Intravenous (IV) administration provides immediate systemic availability.
Metabolism
Minimal hepatic metabolism, primarily metabolized to inactive compounds.
Side Effects
Contraindications
- •Hypersensitivity to cephalosporins, penicillin, or any component of the formulation
- •Previous severe hypersensitivity reaction to any beta-lactam antibiotic
Drug Interactions
Probenecid
May increase and prolong Triphin plasma concentrations.
Loop Diuretics
May increase the risk of nephrotoxicity.
Aminoglycosides
Increased risk of nephrotoxicity when co-administered.
Oral Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect, requiring close monitoring of INR.
Storage
Store below 30°C in a dry place, away from direct light. Do not freeze. Reconstituted solution should be used immediately.
Overdose
Symptoms of overdose may include convulsions, encephalopathy, and neuromuscular excitability. Treatment is symptomatic and supportive. Hemodialysis may be useful in reducing plasma concentrations.
Pregnancy & Lactation
Use in pregnancy only if clearly needed and the potential benefits outweigh the potential risks. Small amounts may be excreted in breast milk; caution is advised during breastfeeding. Consult your doctor.
