Description
Overview of the medicine
Triphin 250 mg Injection is a first-generation cephalosporin antibiotic used to treat a variety of bacterial infections. It works by preventing the growth of bacteria and is administered intravenously or intramuscularly.
Uses & Indications
Dosage
Adults
250 mg to 1 g every 6 to 8 hours, depending on the severity of infection. For surgical prophylaxis, 1 g 30-60 minutes prior to surgery, then 500 mg-1 g every 6-12 hours for 24 hours.
Elderly
Dosage adjustment may be necessary in elderly patients with impaired renal function. Monitor renal function.
Renal_impairment
Dosage must be adjusted based on creatinine clearance. For CrCl 35-54 mL/min, administer usual dose every 8 hours. For CrCl 11-34 mL/min, administer usual dose every 12 hours. For CrCl <10 mL/min, administer usual dose every 18-24 hours.
How to Take
For IV administration, reconstitute the powder with sterile water for injection and administer slowly over 3-5 minutes or as an intermittent infusion. For IM administration, reconstitute and inject deep into a large muscle mass.
Mechanism of Action
Triphin works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located on the bacterial cell membrane, which are essential for cell wall synthesis. This interference leads to structural defects in the bacterial cell wall and eventual cell lysis and death.

Onset
Rapid, typically within 15-30 minutes for IV, 30-60 minutes for IM.
Excretion
Primarily renal, via glomerular filtration and tubular secretion. Approximately 80-90% of a dose is excreted unchanged in the urine within 24 hours.
Half life
Approximately 1.8 hours.
Absorption
Rapidly absorbed after intramuscular administration, peak plasma concentrations reached within 1-2 hours. High bioavailability.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cephalosporins or any component of the formulation
- History of severe hypersensitivity reaction to penicillin
Drug Interactions
Probenecid
May decrease renal tubular secretion of Triphin, increasing its plasma concentration and duration of action.
Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect, requiring close monitoring of INR.
Nephrotoxic drugs (e.g., Aminoglycosides, Loop diuretics)
Concurrent use may increase the risk of nephrotoxicity.
Storage
Store below 30°C in a dry place, protected from light. Keep out of reach of children. Reconstituted solution should be used immediately or stored at 2-8°C for up to 24 hours.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. In severe cases, neurotoxicity (seizures) may occur. Treatment is supportive; hemodialysis may be useful in severe cases of renal impairment.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have shown no risk, but no adequate studies in pregnant women. Use only if clearly needed. Excreted in breast milk in low concentrations; use with caution in nursing mothers.
