Description
Overview of the medicine
Vecef 1 gm Injection contains Cefepime, a fourth-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including complicated urinary tract infections, skin and skin structure infections, intra-abdominal infections, and febrile neutropenia.
Uses & Indications
Dosage
Adults
1 gm or 2 gm IV/IM every 8-12 hours, depending on the severity and type of infection. For febrile neutropenia, 2 gm IV every 8 hours.
Elderly
Dosage adjustment required based on renal function; often similar to adults but with close monitoring.
Renal_impairment
Significant dosage adjustment needed based on creatinine clearance. For example, for CrCl 30-60 mL/min, 1 gm every 24 hours; for CrCl 11-29 mL/min, 500 mg every 24 hours; for CrCl <10 mL/min, 250 mg every 24 hours.
How to Take
For intravenous (IV) administration, reconstitute the powder with sterile water for injection or compatible diluent and infuse over 30 minutes. For intramuscular (IM) administration, reconstitute with sterile water for injection, 0.9% sodium chloride, or 1% lidocaine and administer deep into a large muscle mass.
Mechanism of Action
Cefepime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within minutes after IV administration.
Excretion
Primarily excreted by the kidneys (glomerular filtration).
Half life
Approximately 2 hours (in adults with normal renal function).
Absorption
Rapid and complete absorption after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are achieved shortly after IV infusion.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefepime or other cephalosporins, penicillins, or other beta-lactam antibiotics.
- History of severe hypersensitivity reaction to any beta-lactam antibiotic.
Drug Interactions
Aminoglycosides
Increased risk of nephrotoxicity.
Loop diuretics (e.g., Furosemide)
May increase the risk of nephrotoxicity with aminoglycosides.
Oral anticoagulants (e.g., Warfarin)
Increased INR/prothrombin time; monitor closely.
Storage
Store intact vials at controlled room temperature (20°C-25°C). Protect from light. Reconstituted solutions should be used immediately or stored as per manufacturer's guidelines (e.g., refrigerated for up to 24 hours).
Overdose
In case of overdose, symptoms may include seizures, encephalopathy, neuromuscular excitability, and coma. Management includes supportive care and hemodialysis to remove the drug from the bloodstream.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk in low concentrations; caution should be exercised when administered to a nursing mother.

Lab Monitoring
- Renal function tests (creatinine, BUN) regularly, especially in patients with pre-existing renal impairment or those receiving concomitant nephrotoxic drugs.
- Complete blood count (CBC) during prolonged therapy.
- Liver function tests (LFTs) if clinically indicated.
Doctor Notes
- Monitor renal function closely, especially in elderly or renally impaired patients, and adjust dose accordingly.
- Be aware of potential for neurological adverse events, particularly in patients with pre-existing CNS disorders.
- Consider combination therapy for severe or polymicrobial infections.
Patient Guidelines
- Inform your doctor about any allergies, especially to penicillin or other antibiotics.
- Report any severe diarrhea or allergic reactions immediately.
- Complete the full course of treatment as prescribed, even if symptoms improve.
- Do not share this medication with others.
Missed Dose Advice
If a dose is missed, administer it as soon as possible. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
Driving Precautions
Cefepime may cause dizziness or seizures in some patients, especially at high doses or in patients with renal impairment. Caution should be advised when driving or operating machinery.
Lifestyle Advice
- Maintain good hydration.
- Practice good hygiene to prevent further infections.
