Overview of the medicine
Cefixime is a broad-spectrum antibiotic used to treat various bacterial infections. It belongs to the third-generation cephalosporin class of antibiotics.
Uses & Indications
Dosage
Adults
Typically 400 mg once daily (higher strength tablets are usually preferred for adults; 125mg suspension is primarily pediatric). Consult physician for appropriate adult formulation and dosage.
Elderly
No specific dose adjustment for elderly with normal renal function. Monitor renal function closely.
Renal_impairment
Dose adjustment required for creatinine clearance < 60 mL/min. For CrCl 21-60 mL/min, 75% of standard dose. For CrCl < 20 mL/min, 50% of standard dose.
How to Take
Oral use. Can be taken with or without food. Shake the bottle well before each use. Administer the prescribed dose using the provided measuring spoon or cup. Ensure the patient completes the full course of medication.
Mechanism of Action
Cefixime is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Generally within 1-2 hours for therapeutic effect.
Excretion
Primarily excreted unchanged in the urine (about 50% of absorbed dose) and bile.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% absorbed orally. Food may delay absorption but does not significantly affect the extent of absorption.
Metabolism
Partially metabolized in the liver to inactive metabolites; a significant portion is not metabolized.
Side Effects
Contraindications
- •Hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •History of severe allergic reactions to penicillin antibiotics (due to potential cross-sensitivity).
Drug Interactions
Probenecid
Increases cefixime plasma levels and prolongs its half-life by inhibiting renal excretion.
Carbamazepine
Elevated carbamazepine levels have been reported when co-administered with cefixime, requiring monitoring.
Warfarin and other anticoagulants
May enhance anticoagulant effect, increasing the risk of bleeding. Close monitoring of INR and prothrombin time is recommended.
Storage
Store the dry powder below 30°C, protected from light and moisture. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2-8°C) and discarded after 7-14 days, or as specified by the manufacturer.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Cefixime is excreted in breast milk in small amounts; use with caution in nursing mothers.
