Ciprofloxacin is a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
For most infections, 250-750 mg orally twice daily, depending on the severity and type of infection. Duration varies from 7-14 days.
Elderly
No specific dose adjustment is needed based solely on age, but dose adjustments may be required due to age-related decline in renal function. Monitor renal function.
Renal_impairment
For creatinine clearance <30 mL/min, reduce the dose to 250-500 mg every 18-24 hours.
How to Take
Swallow the capsule whole with water. It can be taken with or without food, but avoid taking it with dairy products or calcium-fortified juices. Do not crush or chew the capsule.
Mechanism of Action
Inhibits bacterial DNA gyrase and topoisomerase IV, essential for bacterial DNA replication and transcription.

Primarily renal excretion (40-50% unchanged drug in urine), some fecal excretion.

Side Effects
Contraindications
- •Hypersensitivity to ciprofloxacin or any other fluoroquinolone.
- •Concomitant administration with tizanidine.
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, increasing bleeding risk. Monitor INR closely.
Tizanidine
Contraindicated due to significant increase in tizanidine plasma concentrations, leading to hypotension and somnolence.
Theophylline
Ciprofloxacin may increase plasma concentrations of theophylline, leading to increased risk of toxicity. Monitor theophylline levels.
Antacids (aluminum/magnesium) and other metal cations (iron, zinc)
May reduce the absorption of ciprofloxacin. Administer ciprofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric emptying (e.g., gastric lavage, activated charcoal) may be induced. Symptomatic and supportive treatment should be initiated. Maintain adequate hydration and monitor renal function.
Pregnancy & Lactation
Generally not recommended during pregnancy due to potential risks to the fetus observed in animal studies. Use only if potential benefit outweighs risk. Ciprofloxacin is excreted in breast milk and may cause adverse effects in nursing infants; therefore, discontinue nursing or discontinue the drug.
