Description
Overview of the medicine
Cefepime is a broad-spectrum, fourth-generation cephalosporin antibiotic used to treat a wide variety of bacterial infections.
Uses & Indications
Dosage
Adults
1-2 gm IV/IM every 8-12 hours, depending on the severity and type of infection. Max dose 2 gm every 8 hours.
Elderly
Dosage adjustment may be necessary based on renal function, often starting with lower doses.
Renal_impairment
Reduced dosage is required based on creatinine clearance; dosage intervals may be extended.
How to Take
Vertex 2 gm Injection is administered intravenously (IV) over 30 minutes or intramuscularly (IM) into a large muscle mass. The powder should be reconstituted with an appropriate sterile solvent.
Mechanism of Action
Cefepime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), leading to the disruption of bacterial cell wall formation and ultimately cell lysis.

Rapid (within minutes for IV administration).
Excretion
Primarily via the kidneys through glomerular filtration and tubular secretion.

Approximately 2 hours in individuals with normal renal function.
Absorption
Rapidly and completely absorbed after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are achieved shortly after IV infusion.
Metabolism
Minimal hepatic metabolism; about 85% is excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefepime, other cephalosporin antibiotics, penicillins, or any other beta-lactam antibiotics.
- History of severe hypersensitivity reactions to any component of this medication.
Drug Interactions
Metronidazole
No significant pharmacokinetic interaction, but should be administered separately due to physical incompatibility.
Aminoglycosides
Increased risk of nephrotoxicity and ototoxicity when administered concurrently.
Loop diuretics (e.g., Furosemide)
May increase the risk of nephrotoxicity with concomitant use.
Warfarin and other anticoagulants
May enhance the anticoagulant effect; close monitoring of INR (International Normalized Ratio) is advised.
Storage
Store the dry powder vials at controlled room temperature (below 30°C) and protect from light. Reconstituted solution is stable for 24 hours at room temperature or 7 days under refrigeration (2-8°C).

Symptoms of overdose may include encephalopathy, seizures, myoclonus, and neuromuscular excitability. Management includes supportive care and, in severe cases, hemodialysis to remove the drug from the body.
